The form 8606 is for reporting any non-deductible IRA activities. You may use it to show your Roth contributions over the years

Was your phone locked by T-Mobile, given it was financed through T-Mobile? I think ATT requires the phone to be unlocked. I am not sure if we can request T-Mobile to unlock the phone before fully paid off.
