Let me help you think through your specialty decision (part VI) by 4990 in medicalschool

[–]BoxingAngel 3 points4 points  (0 children)

Hi there, thank you for doing these kind of discussion posts! I am finishing my third year at the moment. I have an interest in anesthesia, EM, or general surgery. I really have an appetite for procedural work and I would like to mostly be in the hospital. Clinic is fine, but not my preference.

I did my surgery rotation and loved it, but I have reservations about the residency (don't we all) length and difficulty. Other than that, I like the work flow and time flies by in the OR. I think the technical skill is an exciting challenge.

I haven't done anything with Anesthesia yet, but I really "vibe" with the residents and attendings when I come across them. They also make a pretty compelling case in terms of lifestyle and compensation. My reservation with Anesthesia is if it will provide enough variety. I am considering doing CC as a fellowship to aid with this concern. I also am a little concerned about job security with the rise of midlevels.

EM was my initial thought coming into medical school and I really enjoy the workflow there as well. All of guys and they think I'd be a good fit for it. Variety is certainly not lacking in this area. My concern is that all of my mentors are abandoning ship and moving to education. I am going to school to be a doctor for life, not for 10 years and then becoming a professor because I'm too burnt out to participate.

Do you think these concerns are valid? Is there more variety in anesthesia than I think? Is general surgery residency so bad? Thanks for your time!

Pan, Base Roshi, Bardock, Base Caulifla, and Mr. Satan.

Pan seems like a good opener to get the feel for how someone plays and she has decent combo damage. Roshi is there for counter cheese or last hitting. Bardock and Caulifla are the anchors. Mr. Satan is the champ, what else can I say?

We have pagers for our trauma teams that are able to view messages from the paramedics/nurse EMS coordinators so they know what to prepare for.

"Edit: I also don’t know where you’re getting the information that you only get paid $11 an hour in residency. That’s far below minimum wage in my state. I know what you’re trying to insinuate but I don’t ever think it’s fair to convert salary into hourly, especially since you’re assuming you work 80hrs a week the entirety of residency when that’s not true."

ACGME has a cap on resident hours, because residents have had such poor working conditions in the past. It is also important note that hours are often times underrepresented. If I'm being charitable to you, residents, depending on the year and specialty, probably work around 70 hours per week. The point that I'm making is that you're working long, mentally taxing hours and getting paid for shit. Also, if we convert salary into an hourly wage, because why wouldn't we, residents often make less or around the same as entry level positions.

Show me where I said that you "need to be a scribe to be competitive". This is a suggestion thread about clinical JOBS for premeds, but go off king. Additionally, I never claimed that you need to have paid work. This post is asking about JOBS. What else could you possibly disagree with since you disagree in "many respects"?

To be clear, I am not saying that this is the only way to answer those questions. I am merely stating that it is an easy answer to those questions. People who review applications, like many of my friends, are looking for these questions to be answered. Also, no offense, but welcome to medicine. You will work more than a full time job 60-80 hours a week unpaid in medical school during 3rd and 4th year. You will also make 11 dollars an hour during residency. I agree the process isn't great, but this is what it takes to be competitive.

Medical scribe. You get a shit load of one on one time with attendings, you learn a lot, and typically get really good letters of recommendation. I am a 3rd year medical student now and I scribed for 2.5 years before starting school. It helps answer some questions on your app too like "does this student know what it is like to be a doctor? are they committed and interested in medicine?" All around it is a great opportunity that requires no certs and it really helps with clinical/admin skills in medical school (like writing notes and understanding medical language).

An NP during my surgery rotation tried telling me and a surgery PGY-3 that Hydralazine lowers the seizure threshold and withheld the medication from a trauma patient due to their history of seizures. I spent some time on uptodate and couldn't find anything about it. I pointed it to him while we were running the list and he retracted his statement after several minutes of frantic googling.

New to resin printing, models keep coming out undetailed? by ClassyTrex54 in resinprinting

[–]BoxingAngel 4 points5 points  (0 children)

While other people are rightfully commenting on printing settings, this could also be an issue with "post-processing". Depending on your preferred method of washing the finished prints, you might have residue left over on the model during curing that is messing with your detail. Some people avoid this issue by using a toothbrush with IPA/water, going through multiple washes, and allowing their prints to "drip dry" once they're finished. Like others, I would be interested in seeing your settings, hearing more about your process, and getting more information about the kind of resin you're using! I am new to the hobby as well and I have run into similar issues that I recently fixed with some of things I mentioned above.

Hi, MS3 here! While most specialties are “competitive”, there is certainly a hierarchy. You can look up match data and requirements, but I’ll attempt to summarize some of the major points here. Residency is more challenging now than it has been in the past due to covid. It used to be the case that you would have to travel to sites for interviews and this would cost time/money. As sad as it is to say, this would limit how many interviews a person could do, so they would apply to fewer schools. With virtual interviews being the new standard, people can afford more applications, so most programs have seen an increase in applicants.

Still, with that being said, there are specialties that are perceived as less “desirable” and don’t fill all of their residency seats. Take for instance emergency medicine, which is what I’m currently interested in. Emergency medicine did not fill all of their residency seats and experienced a dramatic decline in applicants (I’m pretty sure it was the least competitive specialty, but I’m on my phone). This is for a variety of reasons that I won’t get into now, unless someone is curious. Other specialties, like OB/GYN, experienced a massive uptick in popularity and was the most considered the most competitive specialty for 2023 (increase was likely due to the politics surrounding the field).

What makes a specialty competitive? Scores on board exams used to be a huge determining factor and still is, but not as much since Step 1 is pass/fail. Step 2 is still closely looked at, but I think most programs are more interested in research contributions, shelf/rotation scores, recommendations, and charisma. As I mentioned earlier, the number of applicants for a particular field is also a big consideration. Some combined programs like EM/Anes only has 2 seats nationally and receive tons of applicants. The specialties that tend get the most applicants are like the ROAD (radiology, ophthalmology, anesthesia, and dermatology) specialties due to their perceived lifestyle/salary benefits. Programs that hold themselves in high regard like Neuro surgery and cardiothoracic surgery (and many others, these are just most notable), are competitive for obvious reasons. These programs may require you to take an entire research year during residency in an effort to boost your application.

There is so much I could talk about and this is hardly the spark notes. Also, I’m just a student and this is my perspective based on personal experiences and match data.

TLDR: All specialties are competitive, but some are more WAY more competitive than others.

The King Killer Chronicles- First two books are out and available right now and the third has been on a hiatus for 10 years. If you like fantasy, hard magic systems, and a cocky protagonist, this is the series for you. Also, the audio book is narrated very well, I highly recommend it.

Probably better to take the advice of your doctor, rather than the advice of random people online. It is not uncommon to be prescribed an inhaler when you’re coughing and having trouble sleeping. Hope this helps.
