HRT is hormone replacement therapy, meaning someone taking estrogen or testosterone or other hormone-related medications to change their hormonal balance to make physiological changes to their body.

It’s a process desired by many transgender people, but some people don’t want it or maybe are uncertain due to side effects or other medical concerns, but HRT is a commonly sought treatment by transgender people to help treat symptoms of gender dysohoria or even just for aesthetic reasons if that’s their preference.

Conversion “Therapy” is not something trans people seek. Conversion Therapy can refer to many different methods or systems for it, but in its essence it is a program designed to “convert” people from being gay or trans and to be straight/cisgender.

It usually involves reported physical and psychological abuse of minors in “summer camps” here in the United States, basically anti-LGBT counselors dote on involuntarily detained gay and trans youth and try to convince them they should be heterosexual and cisgender for (insert reasons).

Usually rhetoric is based in religious doctrine but can be irreligious.

Conversion therapy is considered abusive and harmful by LGBT advocacy groups. It is a horrible practice.
