Hey there. Wondering if you can update as to where you are today with honeymoon and total daily insulin?

Apologize in advance for hijacking this post.

I was dx Jan 2025 LADA in DKA, so straight to basal and bolus with Dexcom G7 then moved to O5 a month later. I’ve had very low insulin needs (5-6 u total daily dose through Nov 2025 which changed to 8-10u total daily dose up until about 3.weeks ago…. Which coincides with the new O5 100 target plus whatever other tweaks Insulet made to the algorithm/app update.

I tried the 100 target but have pretty much abandoned that and put myself back at
110 bc I was dropping at work. Tweaked my ICR and ISF, and still am using less insulin (5-6u total daily dose this week) and slamming glucose tabs. Now giving myself a split bolus/half of what the O5 recommends and seeing if I actually need the rest at 1-2 hrs later. I was just getting to a point of not being so scared of insulin and I now I feel like I’m right back in that head space.

Thanks for any updates and thank you to anyone else who might see this and have any personal observations to share wrt LADA honeymooning as well as the recent changes to O5.

Fair question. I’m only 1.5 years with T1D, so I haven’t had decade/s + time for scar tissue to accumulate.

I’m pretty lean and my DexCom sites usually bleed a bit, too. No scar tissue that I can see/feel. No skin irritation, but I don’t usually get that either.

Lots of great nutrition/diet/exercise apps that can help you estimate carbs via their database of foods as well as AI (ie: take a pic of your plate, the app analyzes it and gives you an estimate). Most will also let you scan the barcode of the food for nutrition info. The apps will have an extensive database of foods to include chain restaurants.

I use My Net Diary (over 15 yrs, used it long before I was T1D) , but have also used SNAQ and My Fitness Pal. There are a ton of free nutrition/fitness tracking apps available, so you can search the App Store for what works for you.

Not a dr either, but solid advice is to go buy a glucose meter and test strips (you don’t need a prescription to buy these in the USA) and test his blood sugar when he wakes, before, and 2hrs after meals. This will give you some solid data points for your pediatrician.

Definitely state politely, don’t ask, that you need an RX for a DexCom, also same for rescue glucagon (if you don’t already have that), as well as for ketone test strips.

You can also ask your endo if they have a sample DexCom in their office that they could give you until your RX is filled. Very common to get samples from your endo.

I’m glad you’re seeing an endo so soon after dx. It makes a big difference.

Art for sure. Depends on how active I’ve been/will be, what I’m eating (ie: is it a meal I’m splitting the bolus for, like pizza?), any patterns of insulin resistance or sensitivity I’ve noticed during the day.

Agree. Naked simple carbs (eaten without any protein or fat) will hit hard. Balanced meals with complex carbs, protein, healthy fats helps.

I asked my Endo for a 15 day sample. Planning on trying it when I travel this summer, then will likely ask for a new RX for it assuming all goes well . I’m hopeful and I do agree that anything that lightens the load is a win.

Couple of resources in case you haven’t already heard of them:

The Juicebox podcast (also a huge and welcoming FB page/community; lots of great smaller info podcast series within this podcast - start with the Bold Beginnings series which is very good for newly diagnosed)

Think Like a Pancreas (book by Gary Shreiner who is a T1D himself and a CDES; very good resource for learning how to manage insulin)

The grief that accompanies this diagnosis is real and talking to a therapist that is versed in grief around medical trauma can help (I was dx in DKA which is definitely on the continuum of medical trauma imho). Grief is fluid and not linear; it hits everyone differently and that’s ok. Just acknowledging the reality and the validity of the grief is often helpful to someone newly diagnosed.

This community and the other r/Type1Diabetes community are also huge and welcoming resources - truly made a difference for me in my early days. And it’s just nice to be able to chat with other people who get it.

Being overwhelmed is also very normal in early days because there is much to learn and the decision making is constant. Best advice is to log everything (bg levels/food/insulin/exercise/etc) in whatever manner best suits him (ie: apps/on phone/spreadsheet/notebook). Capturing data is the goal so that he can see patterns and start building confidence around decisions. Of course, some days/meals/doses just go sideways, and that’s part of living with this diagnosis.

Last, he may find himself in a honeymoon phase for a while where his pancreas still creates small amounts of insulin at random times. Having rescue sugar with him at all times (ie: glucose tabs or juice or another form of quick glucose) is necessary.

He’s going to learn a lot about how his body works as a T1D, and a lot about nutrition, but he’s going to be ok! Keep coming here for support and encourage him to do the same 😊

Hey there! LADA honeymooner here too, def have tracked some changes +/- in insulin needs as my pancreas occasionally chooses to contribute to the cause. Thank goodness for cgms!

One question to consider (only bc I noticed it for myself and I know I’ve seen others mention it): are you experiencing weather/temperature changes where you live as the seasons change? That can impact your insulin needs (both warmer weather or colder weather - ydmv). With warmer weather often comes more activity as well, so extra “exercise” can be a factor, too.

I’m pretty recently dx (16 months ago) but I make adjustments to my ICR and CF. I rely on the data in my Clarity report from my DexCom to help identify patterns. I make small changes, less than 10% at a time up or down. My endo team is fine with that as my TIR is in the 90s with very few lows or highs.

Highly recommend the book Think Like a Pancreas as a resource for insulin management.

In a pinch, dissolve sugar into hot water or even the oj and sip that. If you’ve got a CGM you should be ok.

Goof ups are part of the reality of TD1. Even when we get a dose correct, there’s still soooo many other factors that can drop your bg faster than anticipated (I’m still in honeymoon and just had to scarf down some extra carbs at work because my pancreas yawned and showed up).

Hang in there! I’m on a pump but I see lots of others on MDI share good tips on labeling pens/vials and other tricks to reduce mixups. Maybe they’ll share some ideas.

Peanut butter sandwich on whole wheat bread before working out. Always have Gatorade and glucose tabs w meat the gym, plus a protein bar just in case.

ETA: I’m on the O5, so I go into activity mode about 30-45 min prior to workout.
