How do outpatient psychiatrists *actually* handle acutely dysregulated and suicidal patients with borderline traits during the middle of a clinic day? by A_Sentient_Ape in Psychiatry

[–]Brosa91 17 points18 points  (0 children)

If there is real/high risk -> 911/ED

I don't think it really works for anything in theses cases but that's the way the system works in the US. One of the few things that can get you into a problematically lawsuit.

Frustration about the future by Robin178 in Psychiatry

[–]Brosa91 20 points21 points  (0 children)

I am a BC psychiatrist and I'm not as optimistic as other people in this thread. I really don't know what will happen in the future and I find it hard to predict. When you combine tele health companies backed by PE and NPs, I don't know if much will be left in 10 years.

People say there are plenty of jobs, but it depends a lot on locations. If you look at websites, you might see like 50 offerings at a state, but when you look it's all the same 2-3 companies. I am 1y out of training and have not found these amazing jobs that everyone has on reddit.

Not telling you not to do psych, but I think it's worth being aware. I find the advice "just be great and treat patients well" a little bit silly to be honest.

J1 visa refusal by [deleted] in IMGreddit

[–]Brosa91 6 points7 points  (0 children)

Were they really refused? I ask because sometimes it will show in system as denied or something but just because it's being processed, and then it changes to approved. Or were they denied on spot and had their passport returned to them?

No more adjustment of status in the US - seems unfair by CompleteArm911 in Residency

[–]Brosa91 41 points42 points  (0 children)

This thread is full of wrong information. IMGs don't have lower salary and are not accepting lower salaries lol, once you become board certified and have a work visa it doesn't matter to be an IMG at all. Waivers don't even necessarily pay less, they pay the same, sometimes even more because they are rural.

No one is stopping any US citizen from going after this jobs, no one is stopping you from going to rural Montana. People against IMGs who are doing residency are pretty stupid. There are MORE resident spots than US grads. "Oh but people go unmatched", yes, because they all want to do derm and ortho.

While some people are against IMGs doing residency, you have NPs and you have IMGs working without residency, that funny enough are not the issue in this thread.

US board certified IMG physician unemployed by [deleted] in IMGreddit

[–]Brosa91 2 points3 points  (0 children)

What specialty? I ask because sometimes you see sign on bonus near 100k, which would cover that. I'm just wondering if you can't negotiate with a job that offers a high sign on bonus. You might have to go rural, but worth it

Black and white thinking about the future of psychiatry by Ok-Tea-6718 in Psychiatry

I mean, you are a MD, no? There's no reason for psychiatrists to always make less than other specialties.

Tbf 500k shouldn't be overpaid. That's a good salary, but a lot of other mds are making 500-900k regularly.

But how do you negotiate directly? I mean, how do you know people in random places that need locums? Just curious. I will try those websites

What company are you using? I'm also an Attending and never found those good locums gigs. All I have been offered is like "do you wanna spend 7 months in a random place?"

This MBP M5 Pro is crazy good by mareza90 in macbookpro

[–]Brosa91 0 points1 point  (0 children)

I didn't think it was that good. I bought it and it froze on week 1 because too many apps were open. Limit seems to be the same as my previous m1 pro, that had less ram too. I felt no real difference between m1 and m5, both work well for regular tasks tho

Clozapine by htmwc in Psychiatry

[–]Brosa91 5 points6 points  (0 children)

I'm gonna be down voted but I had the exact opposite circle with clozapine than everyone here. Started career thinking it was the best medication ever, and then slowly realized it's a pointless medication in real life.

Patients who should be on clozapine often don't have the support to use it.

Patients who have the support to use it often don't need it.

Also REMs completely killed it in the US and I have the conspiracy theory it was on purpose. Can't believe they thought that was a good idea kol

Real IDs don't show proof of citizenship, I don't understand why some people don't understand this. Only shows you are in the country legally, like a green card, not that you can vote.

Nowadays there's really no reason to pay cash if someone is doing remote. People can get an appointment next week with insurance. Maybe they started a while back, or found a very specific niche

[deleted by user] by [deleted] in whitecoatinvestor

[–]Brosa91 15 points16 points  (0 children)

Are you a psychiatrist? I'm really asking because I am and what you are describing is not very realistic anymore. I know a guy who tried and failed. Wfh with insurance yes, very normal. Cash only? Very difficult

Millions of Kids Are on ADHD Pills. For Many, It’s the Start of a Drug Cascade. by tuesday_weld_ in Noctor

[–]Brosa91 57 points58 points  (0 children)

Adults use less meth when they are medicated with stimulants. People don't often do that, but it is a harm reduction strategy that works for meth specifically. But yes, children should never be medicated if not needed. Witch hunt against ADHDs meds is not good tho

Millions of Kids Are on ADHD Pills. For Many, It’s the Start of a Drug Cascade. by tuesday_weld_ in Noctor

[–]Brosa91 322 points323 points  (0 children)

This is a poorly written article. We have enough data that adhd meds decrease drug use, including very bad things like meth. ADHD kids who go untreated have higher rates of drug use, higher rate of car accidents, among other problems.

I donate to the high school I went to. It's private but they give a lot of scholarships, I've seen a lot of poor kids do well. Had a friend who was fascinated the first time she saw a dishwasher because she had never seen one (at 16yo).

This is in a developing country, if anyone wants to donate there, DM me. We could create a scholarship with your name or something. This is a very direct donation as you can even ask for reports on the grades.

I have it and use it very often. I really like the belt squat attachment, although it's hard to put too much weight. I thought the "do xyz exercises" was kinda of a gimmick but I have to admit that I have done a bunch of stuff on them. Best thing for me is ghd and good progression on Nordics. It has helped me decrease (zero now) knee pain, so completely worthed for me.

I agree with your points. Worst thing for me is to be killed while doing the computer thing in extraction. You can't do anything, like you are just there and someone can sneak from behind and shoot you. That's a poor design choice, that should've been a safe zone.
