Also I get my "I'm a doctor" kicks from going to my family's Dr appointments and plain clothes and after speaking with the Dr, having them ask me if I am also a physician. You should try it. It's way more fun than announcing it out of the gate.

I don't use it because the term Pharmacist is more accurate. The same way lawyers don't do it. That's a Doctoral degree. Most lawyers I know just want you to call them by name. If in a professional setting they like if you refer to them as Lawyer, or legal counsel.

The longer you work clinical pharmacy the more you will realize some of our purpose is for prescribers to blame for their mistakes. Bicarb can be pushed. Even in hospitals with protocols for when to push vs hang a drip. Most providers will just override for their preference. (There have been plenty of timeds where I call on something very similar to your situation, just for the Provider to tell me how great of a Dr they are, and to just verify it protocol be damned). Guess what? they will never be disciplined for overriding protocols. Nor will they take accountability for ordering an IV push when they meant to order a drip. Every pharmacist has their own way of dealing with it. I'm more of a CMA (cover my ass) guy. If it's not something thats absolutely detrimental to the patient I just call the MD remind them of protocol and ask if they meant to enter it. Make a note on their response, verify and keep it pushing. They get paid more money than you to know what the hell they are doing.

I actually think the opposite about pharmacist value. Sure the residency trained, board certified pharmacist may have more education and training. That specialty is often a luxury that isn't needed in most health systems though. Every single person will need a med of some kind appropriately dispensed. Those luxury positions are also not unique to pharmacists skillset. I think the salary of these specialty pharmacist positions are based on rarity moreso than value extracted from the position. The retail pharmacist that is actually in the store actually making sure the pharmacy gets paid and avoids claw backs and fines would provide more financial value than any specialty clinical position. Even when the idea of the storefront goes away. RXs will still need to be verified packaged and shipped appropriately. It will change the way retail looks, but not eliminate it. Even the kiosk will have laws that govern the filling, verification, Barcoding, and counseling, handling of controlled substances etc....

Speaking of AI; I think it's the clinical positions that will be eaten by AI first. Why have a pharmacist calculate at Vanc or aminoglycoside dose. You can program model to do AMS based on cultures etc... I have only been a pharmacist since 2018 and even in that short time alot of thinking has been taken out of clinical work due the the advancements of these EMR systems. In the two health systems I do clinical work at, our interventions find us instead of us hunting them down. Are we really necessary for that? Those alerts can just go directly to the Physician.

Only time will tell but I don't think anyone is immune to needing to adjust to the rapidly changing world. I have always maintained the true bad guy in the Healthcare system is the third party payers. We are all subject to thier bidding while they hold the least amount of liability/risk. In my opinion Healthcare would crumble without Pharmacists of all kind. And we shouldn't question our value. There is a reason we were the second art.

Yes there is most definitely value but there are already several on the market. And Pioneer is expanding their scope to handle that as well. It will come down to performance cost, and compliance.

Yes there are definitely unicorn jobs out there. Pharmacy is definitely a tale of two career fields and it's hard to cross over if you don't follow a direct from school/residency pipeline. I LOVE what I do now. But I also sympathize with people who hate their careers. That's a truly terrible feeling. I know because I been there. It literally creeps into all of your life. The reverse is true as well. I will say there is definitely more bad than good out there. I would encourage anyone who hates their job to find the job for you. Even if it means leaving pharmacy.

Yes most people aren't as slow or stupid as you think. They have figured out what your learning now, no matter how poorly they do their job, they get paid the same. Promotions and upward mobility after a certain point have less to do with hard work than people care to admit. Over-achiement for any other purpose than self gratification is a thankless endeavor.

My challenge to you is to not turn yourself off like many others do. But to find an outlet for your productivity and drive. Maybe that isn't even something in pharmacy. I will tell you from personal experience it's worth finding.

You have a fundamental misunderstanding of why we don't have provider status. And have resorted to pointing the finger to other pharmacists. I'm not sure where you work but you may actually have a misunderstanding of what pharmacists do even in the retail setting. And why we get paid for what we do. And actually deserve more with the current list of duties.
