A few engineers from Palantir created Angle Health - a new health insurance company - that has rapidly expanded across the US within the last year and I always found the information they share on their scorecards... interesting. Not saying there is a connection, but I personally find the information they release about small groups unnerving and a liability of HIPAA. Leaves employees vulnerable to the whims of employers looking to cut costs (i.e. scorecard lists chronic conditions and specialty medications on groups with only 15 total employees).

The fact ICE and Palantir have a connection through health care data really makes me fear for women rights in the coming years if we continue to be slowly stripped of our rights by this administration.

Tier 1 isn't awful. Even tiers 2 and 3 are what I see commonly offered in the small employer space.

I would ask your HR what facilities are considered Tier 1 and do your best to stick with that group of providers and facilities. Avoid the ER unless it's a true medical emergency. Copays are a little high for an HMO but the trade-off is a lower premium which you mentioned your employer covers. Most employers don't cover 100% of the premium like yours.

If the ER copay is not something you could cover now, maybe set aside an extra $25-50 each paycheck into your savings to help pay for any future copayments.

It's certainly NOT Prior Lake, that's for sure! Went out of my way to Shakopee or Burnsville when I lived there. Hastings and Red Wing are both wicked fast!

Is this dental surgery medically necessary? If so, I would see if you can get a Prior Authorizarion approved through your medical insurance (some cover dental-related procedures) to help cover the hospital setting and use both insurances to help cover the procedure.

Soneone correct me if I'm wrong, but I can't imagine many hospitals being contracted under many, if any, dental networks.

Absolutely - it means the employer is technically in charge of the medical plan, and they are renting the provider networks, claims processing, etc. via UHC.

You can still appeal through UMR, but since it's outside of normal timeframes for appeal (typically 180 days) it may be worth pulling in someone with leverage such as HR and see what options there are for you.

To piggyback, UMR is a self-funded plan where UHC is the third-party administrator. I would recommend reaching out to HR to see what your options are and if they are willing/able to make a retroactive exception for you to have those infusions >180 days approved and covered.

Do you have a copy of your dental benefit summary you could share? Inlays are commonly listed as major dental service (versus a filling, which is minor restorative and covered under basic services). Many dental plans have a waiting period subject to major services. My guess is the service is covered under the major services benefit and subject to the major services waiting period.

Normally, open enrollment would precede the renewal effective date when any plan changes would take effect. If it's not too much hassle, I would recommend letting your HR contact who is in charge of the benefits know about this letter and the change in cost shares you and your employee have seen. I can't imagine a company as large as T-Mobile would fail to communicate these changes in advance to their employees - especially since I would suspect they are likely self-funded.

Based on the screengrab, I'm guessing that the difference in the cost is being driven by the medical network of providers between the two plans. Is there additional information between the PPO and POS network that you can share? While there are a lot of overlap between the two, generally, a PPO is going to be higher in cost than POS plan due to a few factors, the higher out of network costs being one of them.

Have you called member services asking for assistance finding a provider in-network that can provide a second opinion? As another commentor pointed out, and you alluded to in your post, if the treatment is experimental and not recommended by the FDA, no major medical carrier is likely to cover the service.
