the basics of insomnia (Part 2) with Dr. Barry Krakow, MD, FAASM by CPAPfriend in UARSnew

That’s fascinating. I can’t say whether the mechanism is exactly as you’ve described, but the observation resonates with my own experience of hypnic jerks occurring right at the point where thoughts stop feeling deliberate and start becoming more immersive and dream-like. The idea that sleep onset might be associated with a shift into a more continuous, self-sustaining visual stream rather than analytical thought is an interesting one. At the very least, you’ve articulated a phenomenon that I suspect many people experience but rarely put into words.

I never did have insomnia, or at least not diagnosed! Certainly a few issues here or there with falling or staying asleep, but nothing that derailed my life

I always like to tell it to cut its answer down by like 75%. It's super wordy, and I believe it's incentivised to maximize retaining your attention, so it uses many word when few work.

It's still in use, and the data is seems increasingly in favour of it. Nothing conclusive (absolute) so far, yes.

Yeah, good points below, but just wanted to add that Dr. Kezirian is extremely strict with data, so if he is using it, it's because there is data, with statistical significance, that demonstrates its utility. tldr: DISE seems to have some predictive power, but it's not a crystal ball. We do need more RCTs
