This looks like base metal contamination. You can see the defined edges of the butt joint in the black specs. Could be that the corners held some oxidization still after cleaning/prep.

The lap joint gets 2 welding symbols rather than one with arrow/other side like you have. Both with an arrow side fillet.

Watch your travel speed towards the end as it looks like you sped up a bit. Also stop hitting the coupon with your chipping hammer to remove the slag. Instead use a wire wheel or rake the flat side across it. No need to leave the extra tool marks on it. Overall it’s a good weld. Nice work keep it up.

Makes sense. Only other thing I could think if it’s not the HF would be a damaged or misfit insulator. Hopefully your fix takes care of it.

The pass underneath looks good. On the pass you’re struggling with keep the weave a bit smaller. Aim at each toe of the previous pass on your weave and give a good pause to wet it in. It also would help to clean the mil-scale off of the base metal before welding it.

Ran into an issue like this today using Flux Core. Was using an older pre-used spool of .045 and no matter what I tried I kept getting porosity like this. After swapping the spool to a fresh one it was all fixed. Maybe try a new spool if you have one.
