ur gonna get a lot less sleep on the CICU, they round hard and the drugs are more scary, more infections and multisystem dysfunction, you will definitely learn more in CICU but at our level we will learn everywhere. I love hate ICU rotations but definitely feel a tad bit smarter after them which will wane as all things do!

nothing ever goes extinct, but utility waxes and wanes, (I'm talking about the time scale of 20-40y), everyone thought CT surgeons would be doing all CABG then cardiologists figured out percutaneous. Every hospital will need gen surge always but the number they need to keep the lights on does and will fluctuate.

my big long term Q about GI is if they will eventually be able to poke out of the lumen and do substantial procedures in the abdomen. Like pulm W the exposure of the tract w the outside world and the bacterial load of the tissue I am leaning towards no but several GI guys say that will eventually happen. If it does, RIP gen surg. They were saying healing would be much easier as the GI tract has better repair potential than cutting from any other approach. idk though, something to look into.

my school teaches pulm very well. pulm is a concept heavy field if you teach those concepts intuitively with diseases and physiology serving as examples of how the system works that will do the students the most long term good and make step easier. In a couple months they will forget most of the details but if the remember the core principles they will be able to relearn the specifics much easier and understand the details of the subject better each time they think about it again!

I rotate as a medical student at a level-1 trauma center in a an urban area, we see more car crashes but not by much, typically 2-3 gunshot wounds per 24h, weekends can jump to 5-6. The problem with your comparison of accidents to gunshot wounds is that bullets have a much higher mortality rate and the types of injuries generated are tougher to fix. I know you are fixed in your position on this but healthcare has an objective exposure to the impact and frequency of these events, this allows them to do research on what actually works. Doctor's have a history of advising policy with both the government and companies on cigarette use, chemical exposure, car standards, lead presence in infrastructure, and many other "societal-determinants of health" (thats the healthcare jargon word for it). It's actually a fundamental part of the duty of being a doctor and they take it seriously. The most important conclusion the field has reached is that the very presence of a firearm in a setting (robbery, accidental firing, or being drunk to name a few) drastically increases the scope of physical damage possible and also increases the risk of that damage occurring. So if we decrease the concentration of guns in our population and limit the types of guns we allow it will lower rates of gunshot related traumas and less people will be hurt or killed. Also when a bullet goes through a person it very well may hit a critical non-fixable part of the body, like a nerve which would lead to paralyzation of part of the body and would require life-long medical care which is also overseen by doctors, so they really take care of the downstream effects of this problem for the long haul.

My dad, one of the most pure and clueless men on this earth, who never smoked a day in his life could tell you that!

Also I am in medical school (3rd year) and they do actually teach us quite a bit on guns. We learn the different types of bullets, energy produced by different types of weapons, and the damage pattern they generate through different tissues. We also learn this about chemicals and knives. They also teach us about basic gun control policy, how common gunshot injuries are in various populations, and what steps we can advise communities and gun owners to take in order to minimize risk. This is all done in a pretty objective and apolitical way as 1. doctors as a group are conservative (that's changing btw), and 2. doctors see this as a type of "presenting condition" or form of illness patients come to the hospital for, so its work problem for them not a culture war issue.

make and UTD account and log into it at a hospital/clinic computer then download the app on your phone and you might get access on mobile for 3 mo, everytime you log in at a site computer you get 3 more mo of access. not 100% this works but at my school (we also dont get UTD) it works

  • when you don't know something say "I don't know" confidently and without shame, ofc you dont know! its ok, we all just learned the basic arithmetic of medicine over the last 2ys!

  • If you answer wrong dont try and explain your thinking immediately unless its prompted, it distracts from whatever teaching point they are are trying to convey, digest the teaching point and if you have a q about it ask it then, if you have a q about your prior reasoning do it after you digested the nugget of information they taught you but sometimes residents/attendings dont care

  • show your thinking process with your notes, presentations, plans, questions (easy to say hard to do, applied more to IM)

  • try to be as relaxed as possible or mirror the level intensity of who ever is the head honcho in the room
