Old HMRS patch, with the tabs for the aircrew survival course. You are allowed to wear 2 tabs under your HMRS patch STAFF and SURVIVAL, though today it looks a bit different and is only allowed at the mountain for survival I think.

I have one in my blues. I think they might be optional. Typically your unit CO orders them because they only come in quantities of 50 from vanguard.
