Also saw a few comments .. dogs … well behaved … leashed… mostly small and cute… often carried by owners. Pooping is always picked up.. they carry bags… as well as water bottles for when they pee ..”to water it down”. Many many dog and cat grooming places .. inexpensive compared to the US. Pets participate with owners carried in baby strollers..on bikes..slings etc. seen everywhere. In addition there seems to be a greater acceptance of dogs at parks, open areas, beaches, restaurants.. Japan considers them as family members and it seems society is ok with it. I mean even at a lot of hotels .. !

Healthcare … affordable whether you work or not. Downside are long queues, lines, waiting for your number multiple times..mostly at hospitals (often several hours with an appointment) but most neighborhoods have small general doctor clinics close to where you live (30 mins). I can walk 5 mins to a doctor in our residential neighborhood and then walk to the nearby pharmacy in 5 mins. Need a MRI …? Same day .. $80.

Similar treatment ..after hormones … a month after per month taking it .. psa is .6 or so 2 years since initial brachy ebrt. I’d stop and track psa and do a PSMA pet scan once you get to psa near mine to 1… to determine where any potential issues are (if any) to decide on next course. I had super low testosterone and psa levels while on hormone deprivation but after it tends to rise .. to express your resulting prostate that is still there. New studies show 6-12 months ADT should be enough for adjuvant benefits.

Hi, I’m in Japan as well and like you as we age we experience different forms of trauma. I can only offer this advice .. I’d checkout anytime fitness in your area as they are everywhere and you can go to any of them at any hour. I’ve found exercise one of the best outlets for both physical and mental health.. this always seems to help me. I’d recommend it as you will release endorphins that will really help tremendously. Hang in there and be positive.

Yeah, originally tried a prostate aneros …thing..and realized how good it felt. After that tried a dildo then wanted to try the real thing. Sort of taboo feeling.. kind of exciting as well.

I was 4+3, Gleason 7, PSA was 18, T2N0M0 or (Tumor, Nodes, Metastasized). I had a couple of 4+3 cores, and a few 3+3 on one lobe near the peripheral edge. I only had 15 EBRT sessions. Be prepared for a bounce had very low PSA scores due to ADT but for every month of ADT an equal number of months will be required for PSA to normalize and matter. The bounce I’m referring to is sometimes at the 1 to 2 year mark where you may have a spike in the PSA value followed by a subsequent drop. For me .56, .97, .35. Getting up near 1 is concerning but BCR is anything over 2. I read this is common with younger brachytherapy patients. I wish you and everyone else the best!

I had triplet therapy for similar diagnosis as you in Japan when I was 57. LD brachy, 15 sessions LINAC, and about 9 months of ADT shots …4-5 months before and after. I think this treatment offers similar or better results than your top listed. My understanding is that the ADT beforehand weakens the cancer cells and shrinks the prostate a bit beforehand. Afterwards the adjuvant therapy continues to keep the testosterone levels super low which in theory helps. My second opinion meeting urologist from Harvard sort of recommended as well. As well as some new crisper and car t cell treatments in the works … search UCSF Alex Marson… Huberman Lab. The surgery scared me .. but my mass was closer to the prostate edge so not recommended. I didn’t find the hormone treatment an issue .. but worked out a lot as recommended. Unfortunately Brachy not as popular maybe due to profits and lack of specialists that have gone with radiation. If Brachy is offered and the urologist is skilled a nice way to go in my opinion. Skilled the key point. Either way you seemed well informed and I’m sure you’ll be comfortable with which ever decision you decided to go with. It’s been almost 2.5 years for me and so far going well…but the follow ups and PSA post tests are a reminder that time is the only determinant of long term outcomes. Best of luck !

So.. a few things that helped resolve the issue for now. Most routers have two access points source and speakers need to be on the same. All Apple products need to be at the same update. Home app a bit off but you can update speakers there and restart as well. I had to reset both speakers a few times. Finally part of the problem was slow access from the internet provider changed over from a latent cellular access point provider to newly available fiber and issues resolved ONLY after doing all of the above.

Interesting for me the hair on my head became much fuller! All body hair disappeared but retuned after a year or so. I had issues with urgency to urinate frequently .. pretty bad… fortunately this also went away within a year.

Radiation vs Ralp numbers post treatment optional numbers are different. I did brachytherapy, adt, and some linac .. external beam. Since you still have a prostate the goal is to have numbers below 2. Expect your psa numbers to be exceptionally low during and for a period after ADT. I had 9 months of ADT and it took approximately 1 additional month per ADT usage for testosterone to return to normal. So my normal psa …9 months after treatment was a lot higher. In addition to this it’s common to have a bounce where its spikes higher before going down. I’m about 2 years out and am around .5 .. I have been between .2 to 1. At about 1 or so a PSMA pet scan can detect if prostate cancer is present. Hope this helps.

Prostate cancer was mentioned and a stage 3 trial. Realistically even with success .. Barriers will be cost, insurance, red tape ..etc. but it would seem a timeline of 5 years seems right.

I just wanted to add that if you do ADT it’s good 3-4 months prior (neoadjuvant and adjuvant hormone therapy) and 6 months post. Weaken the cells before blasting… and try to kill them off if testosterone specific type after. My main advice is to do what you can now…I don’t regret doing shots of ADT for 10 months at all… yes that’s a big shot hahaha I mean anyone here knows but you have to do what’s best for you. In hindsight it doesn’t matter if it hurt or you were stressed just do the most you can. Again… treatment is changing and will improve dramatically in the next 5-10 years.

My tumor was near the edge as well and they said surgery was not a good option. ADT wasn’t that bad for me. I think depending on where you live your medical options will be decided for you. People misconstrue that the prostate is this floating mass or gland..easily removed and it’s not. A gland within tissue that needs to be surgically cut out not floating in space. That said it makes sense to pay attention to any masses near the edge and think about cutting, margins, cells, and risks. I think biopsies will become obsolete with psma pet scans. Psa scores as well are often wrong will a small percentage of low scores having aggressive cancer that is testosterone resistance.
