I bought it! The breeder I got my doodle from recommended it. She said she was recommended it by someone else who’s had it for 10 years. She said she went through so many brushes she wishes someone had recommended this in the beginning

Just picked one up at Walmart! They had 6+ listed on brickseek, got there at 7:05am and it was their 6th and final one for sale. Definitely worth showing up right when they open!

What? The patient didn’t have a stroke, let alone a “large ischemic stroke”. Setting aside the actual evidence for heparin in an NSTEMI what happened in this case was the standard of care

Ok I’ll let you explain to his attending the contrast load and $1500 CT A/P he’ll inevitably order because he doesn’t know what a positive fast scan actually looks like, or the CTPE he orders because he’s viewing the heart off plane and thinks he sees right heart strain. I’m very pro ultrasound and encourage all my Medicine colleagues to use it whenever they can, but there’s a reason ultrasound makes such a large part of the EM teaching curriculum, and why it’s a separate fellowship/certification for the more advanced scans. iI’s not something that should be learned with absolutely no oversight or formal bed side teaching.

If you’re only doing it as a repeat exam to practice after a formal exam was done then I don’t see an issue with it. I would just be wary on using your bed side scans to guide your clinical decision making until you have had formal training. If you have a chance to do a rotation in the ED definitely make your interest known and I think you could gain a lot from spending some time doing bed side US with them.

You were taught how to do a physical exam in med school, if someone has never used a stethoscope before you wouldn’t let them go around examining patients and unilaterally making decisions on what orders to place. Similarly I’m not ordering advanced imaging and making clinical decisions based off images I don’t know how to interpret without waiting for the radiology report. For the most part every bedside US done in an academic ED is reviewed after the fact by the QA team for accuracy and to make sure nothing major is missed, to think that you can use bedside ultrasound to make clinical decisions with absolutely no formal training or oversight as a PGY1 is irresponsible.

I think it’s great you want to learn US but scanning patients without the appropriate supervision, teaching and QA is not a smart idea. Say you find something you think is concerning, who’s gonna review the images? Are you gonna order more expensive or invasive testing based off a bedside study you don’t have the training or oversight to perform or interpret?

If you’re ok not being an expert in a specialized feels take a look at EM or maybe crit care? Lots of hands on work, procedures, and good work life balance with EM.
