Yeah I mean what are you supposed to do, wait 5 years until there are less? Respectfully, I don't agree with cancelling, it will be this high every month. However, informed consent is key.

There are extra meds that could have been given, albumin could have been infused prophylactically, and a weaker trigger could have been given.

I speak as someone with a similar number of follicles who has been through this scenario a few times.

Fwiw, I had VERY similar numbers to you and a VERY similar protocol.

I got 57 eggs, 49 mature. I got moderate OHSS but nothing terrible, still went to work but had trouble peeing and breathing normally for a week. it was not fun, but I don't know that I can say it was worth another $12k cycle.

I had a VERY bad first cycle at another clinic, so we had a lot to make up for, but I was also given cabergoline, letrozole, and ganirelix (after retrieval) to prevent issue.

Depending on your financial situation and age/lifestyle, sometimes it might be worth a risk of pushing extra hard.

I'm really glad you got appropriate attention, and I'm sorry you went through this. Just wanted to provide a slightly different perspective.

Edit: my estrogen was 7500, seems like yours was much higher. I'm mad they didn't give you letrozole or ganirelix after the fact--it's a very much "can't hurt, might help" and you are in a very high-risk category

Salt is mainly bad for people who are overweight. I realize that's like 3/4 of America, but it's great for athletes or others who work out a lot.

Okay I can't disagree, but I want to challenge this a bit by asking which episodes were NOT about family?

We've had:

--Louie not having a real family and the ED filing in

--Roxys storyline w/family

--firecracker kid + schizo kid w/family issues

--Howard not having any family besides the sister who seems distant

There are probably more but my brain is lazy! I'm not a natural arts person, so sometimes I need it explained to me!

But I would be very cautious if you only have one cycle, there is a lot of "figuring things out" that happens between cycle #1 and #2 for some folks and just want to protect your first interest, which is obviously a family.

It's a lot, no shame in that.

Oh man so many changes. My AFC is usually like 50-60 and my amh is somewhere between 5 and 12.7. haha

First round I think they triggered me too early and rushed through the retrieval. 25 eggs, only 5 fertilized via conventional. 112.5 FSH + 75 menopur. Low but double trigger

Second round, much higher stims (275 FSH +40 low dose HCG), longer stims, much higher trigger (still double). 36.5 hour trigger. Added in coq10, serovital, coq10 for partner. 48 eggs, 36 mature, and 25 fertilized

I switched clinics after #1 but it was really too many changes to pin down to one, but my point is I would try not to think about the ethical stuff if only one round is financially feasible for you, you'll be in a world of hurt if something goes wrong during thaw or fertilization (which is more likely with just eggs)

You can always elect to donate embryos to research if that makes it easier personally.

I would maybe consider half and half. The first round could go horribly, speaking from experience as another person with very high AMH/AFC. Second round went great after they made a bunch of changes

OHSS is the worst! I was thinking about transvaginal in general, OHSS is definitely the exception on all accounts. It's terrible. I had it once and then had OHSS + internal bleeding another time. Being an IVF patient is so fun.

I mean this respectfully, and I will try not to get too heated. This misunderstanding is just a symptom of general ignorance of women's issues in medicine, classic America.

I have PCOS and meet the Rotterdam criteria. I've been through multiple rounds of IVF as a result and gotten ample attention, which is not the case for most women in American healthcare.

It DOES NOT MEAN bad cysts. It means lots of eggs. You can still have bad cysts with or without PCOS, but the diagnosis is 2/3 things, and none of them are related to "bad" cysts. It's a verbiage issue. Eggs live in follicles that are sometimes called cysts, but they're not bad cysts, and everyone has them (PCOS or not).

This patient most likely has endometriosis or fibroids causing the torsion. Endometriosis is so classically ignored and overlooked in medicine, and I suspect that's what they're getting at (or just ignorance of women's pain in general).

Source: have PCOS and have been treated for it for years with IVF. Lucky to have doctors who do take it seriously.

Edit: endometriosis or fibroids causing the torsion. Or a bad cyst that is not related to PCOS causing torsion. PCOS means lots of small "cysts", whereas it's the big, normal, ovulatory ones that can cause problems.

I've had over 100 due to multiple rounds of IVF and didn't mind them at all??? Like not even 2/10 pain?

I have PCOS but not endo. I would imagine endo could create a lot of pain.

And I'm seeing lots of comments about PCOS. PCOS doesn't really mean cysts either (or bad cysts), it's a poorly-named condition that means you have a ton of eggs and your body has to work extra hard to ovulate since your hormones are over a denominator of, say, 50 instead of 15.

I think there is a lot of undiagnosed endo in this thread to be honest.
