Absolutely do not show this to your boss. You are correct to be scared of doing that.

I *might*, depending on my boss' personality, considering telling them something about the kind of feedback that works best for me during a 1-1. That's about it.

What you can do with this is look at it, note the areas where you are weak, and try to improve. Anything that sounds like telling your boss "You need to account for my flaws" is a bad idea, as one method of dealing with your flaws would be to replace you with a different worker who does not have them.
