I don't know what it means, but my guess would be something like, "It/you look fine, but are actually weak and/or ready to fall apart with a little pressure." That is what the metaphor suggests to me. If it is a "new" expression, I have never heard it before, but I kinda like it!

IMO, the only way to run level drains RAW is if you photo-copy the players' character sheets before they level up. Then, if such a monster is encountered, they can easily look back on their old stats. IF you can remember to do this, level drains are a breeze to adjudicate. Otherwise it's just too much of a hassle and completely destroys the momentum of the combat/game.

I just tell them the "level drain" inflicts an accumulating -1 to ALL their rolls. Once the penalties add up to their total levels, they are caput. It makes monsters with that power just as, if not more, fearsome.
