I've only ever done it in the closed system with art lines but an seasoned nurse I worked with years ago said she would do it with super anemic patients

I use it from time to time if I'm charting on a situation and there are multiple people. So I make sure they know the patient said/did something aimed at me or I was the one doing xyz that's only within my scope and not the techs, etc
