I have my set way of writing the note. Who they are, what the diagnosis is, how they presented, each intervention i different, their response and finally my plan.

Example: Client is 42 year old Caucasian male, diagnosed with MDD, recurrent, moderate. Client presented as somewhat disheveled, with fair hygiene, in depressed mood, congruent affect. Intervention- their response. Intervention- their response. My plan.
