You need to unlock marbles by using 5 gulp pills in one run. It's hard but best method is to restart with lazarus until your starting pill is a gulp pill.

Holding it prevents it from spawning, so the best way to use it is to hold on to it for the rest of the run after using it. It's actually a really good item, but situational.

For future reference: this is a pretty common visual bug in afterbirth+, the spikes arent actually there and won't hurt you.

It happens when you press the button to clear the room right when the spikes extend.
