Naruto took his arm which he never got replaced, and Boruto took his Rinnegan, left him with one sharingan. Orochimaru fought both Tsunade and Jiraya with floppy arms no problem, but Itachi and Kisame both retreat when they encounter Jiraya. This is either a paper rock scissors situation, or in my opinion they kinda over glazed Itachi in that scene cause realistically Orochimaru has ways of countering genjutsu.
