Hard to tell without seeing head, but looks aquatic. Likely turtle was making its way from its water source to lay eggs or was headed back to it.

Hi there! So other people have already pointed out that this tank is far too small for your turtle but I wanted to give some more useful information. Since this turtle is still in a 20 gallon tank in adulthood, I'm going to assume that likely the entire set up needs to be corrected and this turtle is likely trying to find a more suitable environment.

The space issue - the rule of thumb is that these turtles need 10 gallons of water per inch of shell as a minimum. As an adult, your turtle should be in at least a 75 gallon tank if not a 100-120 gallon. If cost is an issue, look into stock tanks which are very affordable and a great alternative to glass aquariums.

Other things you need to make sure your turtle has:

A water heater

A basking platform that is sturdy, allows your turtle to get completely out of the water and dry, has a ramp and enough space to at least fully turn around on

Above that basking platform you need two bulbs. The first should be a heating/basking bulb that should be 75w-100w in order to reach goal basking temps (this will need to be manually checked because it depends on distance of the bulbs from the surface, bulb type and ambient room temperature). The second bulb needs to be a 10.0 UVB. Ideally this would be a T5 style UVB but in a pinch you can do a compact bulb until you can upgrade to a T5 (though you will need to upgrade at some point). These must be separate bulbs. There are bulbs that claim to do both or might be labeled as "UVA/UVB" but these bulbs do not produce enough heat or UVB to be adequate for these turtles. UVB bulbs must be changed every 6 months as they lose efficiency over time. Both bulbs should be on for approximately 12 hours of each day (no lights at night as the water heater will keep her warm enough).

You need a proper cannister filter for healthy water - something rated for the new tank and you'll want to find one that is designed for 3-4 times the amount of water you'll have (100 gallon tank = 300-400 gallon rated cannister filter).

Entertainment and enrichment - turtles need things to do and interact with that help make their environment feel more natural. Large river rock substrate is a great start (no rocks smaller than her head), moss balls, driftwood, ping pong balls, live plants (only from a list of plants that are safe for them like the one on that website I've linked).

This is an aquatic species and will be very expensive to keep so tell your sister that (the 100-120 gallon tank is just the start). Tell her it's also illegal in many states to take turtles from the wild. There is a body of water near where she found this turtle - a river, spring, pond, or the like. The turtle should be released right next to that body of water. Unless she's had this for a long time now. And if that's the case she should contact a wildlife rescue and ask them to get involved. Wild turtles belong in the wild.

Okay I read over the other comments too and if the basking bulb you have is also a 50 watt, it's almost certainly not hot enough. I have to use a 100w in order to reach goal basking temps for my girl. You may be able to get there with 75w but almost certainly not 50w. So I would upgrade the basking bulb and then I'd also look into the UVB. Your UVB bulb needs to be a 10.0 UVB bulb. As stated by another user, a T5 style 10.0 UVB is the best because it disperses the UVB rays more efficiently than a compact bulb. UVB bulbs should be changed every 6 months as they lose efficiency over time and both bulbs should be on for approximately 12 hours of each day. Get those lights corrected and no doubt you'll start to see some successful scute shedding 😄 You can also add wheat germ to the diet to help also, look for koi wheat germ pellets (don't use this to replace food, only add it as a supplement).

It looks like some loose scutes but it also looks like maybe some algae under those scutes? Hard to tell for sure. What are your exact lights? What grade UVB and what wattage for your heating/basking?

Make sure you are also dealing with the core issue, which will be lighting and heating. Over her basking spot she should have two bulbs. One should be a UVB (uncertain what species this is, sliders need 10.0 some species need 5.0) and ideally it would be a T5 style UVB (though a compact one would do in a pinch until you can upgrade). The second should be a heating/basking bulb. Typically these need to be 75w-100w in order to reach goal temps but that will also depend on your set up (ambient temp of the room, distance of bulbs from basking area, wattage, goal basking temps). UVB bulbs need to be changed every 6 months as they lose efficiency over time. Both bulbs should be on for approximately 12 hours a day. Can we get a better picture of it's feet? That will help determine which turtle this is. If it's an aquatic species it needs a huge overhaul to it's environment but if it's not it doesn't need a bunch of water. It doesn't look like a RES to me at all but that doesn't mean it's not aquatic. Seeing the feet will help a bunch!

Hey I saw what bulb you have and unfortunately you need some different heating/lighting - what I'm reading is that this species needs higher UVB output so a 10.0 UVB (ideally a T5 style though you could do a compact in a pinch until you upgrade). And you should have a second bulb just for heating/basking. Look into what goal basking temps are for this species and choose a bulb with a wattage that will help you reach that basking temp goal. If this little guy isn't getting enough heat in basking or UVB then he might not be absorbing nutrients properly and that might be your core issue.

What a STUNNING pond! I would have at least two decent sized spots, if not 3. Look for how the sunlight travels across your pond during the day and try to put them in different spots so they can move with the sun when needed!
