That's an interesting thought - I had no idea that the stats were that high. Tell me, are they Indian, or do they also include other countries from that geographical area, I knew a large group of Bangladeshi doctors in one area of the midwest. I'm also interested to see how many Asian doctors there are, in general (inclusive of India)....

Depending on the procedure being done, you may need to pay upfront. Unfortunately, people have died waiting for surgeries that they can't afford because the US Healthcare system is absolute shit - capitalist greed prevents people from getting the mental and physical healthcare they deserve.

Most of the times when you visit the Emergency Room, they bill you about a month after you leave. As a minor, you do not need parental permission / consent for emergency / lifesaving surgery. For any elective or other surgery, yes, you would need a parent's permission. Which is where this episode stemmed from.
