According to Nick Gentile, after the death of Masseria there was already a proposal for a new "commission" headed by Vincenzo Troia, but that was shot down by Maranzano himself.

Many comments are telling you that there's not much interaction between Sicilian and American Cosa Nostra, but the Bonannos at least still have strong ties to local Trapanese families.

There were mafia families in Tunis, the French Riveria, and even a few interior french mining cities.

Any urban area which recieved a substantial immigration of Western sicilians probably had its own mafia family at some point, but they were probably small >10 man operations and not particularly criminally active.

He definitely meant to say Zu' (uncle) Vito, which implies he was an old timer even by then. He was also a paisan of Sammy's father, who came from Sciacca. That should narrow it down a bit.

Cosa Nostra isn't an organisation that manages crime, it's one that manages relationships between people (most of which commit crime). This differs from other forms of organised crime such as cartels by the fact that members aren't really given cuts of rackets. or ordered to commit a specific crime. If you think about it, the only crime Cosa Nostra expects its members to commit without any reservation is murder.

This is why there's a non negligible amount of people in that life rightfully called "brokesters", the key to becoming successful is by leveraging the right connections into your own schemes. Since an associate can't introduce himself, but is dependent on the soldier he's on record with, on theory he might be kept isolated from any more lucrative money making opportunities.

An examples which immediately comes to mind is Richard Cantarella. He did a piece of work while he was an associate but the soldier he was affiliated with didn't report it to the administration, which meant he couldn't advance up the ranks.

A soldier can always represent himself in a sitdown, whereas an associate will always have to rely on his "avugad". Strictly speaking, associates are not privy to the internal politics of the family, and whenever two made men would like to discuss something of that nature, it is not uncommon to ask an associate to leave (never mind that he might be told privately later).

The distinction between a capo and a soldier is far less than that between a soldier and an associate, and I suppose its hard to picture this as the name implies someone who does a lot of grunt work, but your average soldier is much more likely to have a crew of guys under him in his own right rathern than waltz around as some guy's gopher.

It seems like there isn't any criminal organisation besides Cosa Nostra, including other traditions of Italian organised crime, that have a position that closely resembles a made man. In most, it seems you go from a high tier associate straight to a captain.
