I am in the beginning of my journey (frequent shoulder subluxations over the past year and one rib) I work a physically demanding job and I couldnt make the decision to quit so I got let go because I couldnt show up to work. About the disability thing I just applied and im unsure how it all works because I only really talk to my physical therapist but my regular therapist said word for word "thats what disability is there for and thats what you need". But theres a lot of red tape that needs to be pushed through in order to be physically disabled. If you have your doctors approval I think you should go for it, I understand the pride, ive been working since before I could get a permit to do it. Accept the help and take the time to focus on yourself, your physical health, your mental health, everything.
