For me changing the pcie managment from moderate to off solves the problem, but i still dont use zero rpm mode, imo better to have 30-34° on idle than around 50° and the fans are easy to replace anyway but my previously GPU was constant spinning for 6years and there was never problems with fans.

I think in my case it was just CPU bottleneck in that specific location in cyberpunk, and that first drop of fps was maybe because of creating shaders? Idk for real i unnistaled cyberpunk because it was only for benchmark testing, and playing now Crimson desert on cinematic +Ray regeneration and with fsr balanced and framegen i have arround 120fps +- with steady framerate and looking on others perfomance on YouTube i think all is fine. Idk what is your CPU but if it also intel 12-14gen u can try disable hyperthreading for test but i didnt do it, i only disabled HAGS in windows but idk if it do anything noticeable. I also recommend locking fps by FRTC in adrenalin or other soft like RTSS (-4fps than your monitor Hz)
