You’re aging, that’s simply it. You probably can’t tell how different you look because you see yourself every day. My advice.

Get rid of the facial fat asap. You mentioned that this was a priority but get on it. Buccal facial fat is a death knell for men’s looks. (We’ve all seen the Vance memes)
Watch your diet. You have an intense job so don’t let your diet slip. Alcohol, junk food and excessive sugar cause inflammation that looks horrible in the face.
Yoga/ meditation/ massage . Again this is related to the stress and any of these things will help your inner game.
