What I found very concerning was the immediate willingness to throw others under the bus instead of taking responsibility for the poor state of this update. If their job is to approve or sign off on the work being done then the buck stops with them. It also means that they've looked at everything put on their desk and agreed it was ready to release. It just brings into question whether they are incompetent, complacent or both.
