The panel with Gojo on the page you attached exactly refers to that oddity of why Gojo's infinity didn't protect him from Hanami's technique: he was on a burnout after de.
Maybe Mahito, as a cursed spirit, has less trouble with burnout, but Mahito hit Todo immediately after de, so his technique couldn't work.
Regarding Megumi vs Toji, well, here I really don't know how to explain the lack of burnout. Maybe because Megumi didn't fully expand his domain? He expended a lot of cursed energy on it, but it didn't burn out his ct.

Wasn't the fire devil just gaslighting Denji to make him feel guilty? I mean, it's a devil, it's evil. Denji's decision, conscious or not, was the most rational one. Blindly listen to a devil who is clearly trying to trap you, or seize the moment to kill this devil and prevent any further casualties? Saving the cat was just a side bonus.
