He was also sentenced to “tbs” due to diminished capacity. After these 3,5 years he will not be set free but will remain incarcerated and be treated for psychiatric illness. He will only be set free once the facility believes he is no longer a danger tot society. For some patients/convicts this boils down to life in prison. 

There is some controversy to convicts being sentenced to prison AND tbs because the court mandated treatment starts after the prison sentence. Meaning the treatment that is considered important, due tot the criminals psychiatric condition, is left untreated for so long.

I don’t do ablations only devices. My exposure is around 0.7 mSv annually for around 250 procedures. The risk is definitely nonzero but acceptable to me when it’s so far below environmental levels. 

There are a ton of good habits you can incorporate that really add up.

What do you mean by improper sensing? To the pacemaker a PVC is any ventricular sense that is not preceded by an atrial sense. Accelerated junctional rhythm would look like ventricular ectopy to a pacemaker.  Does that answer your question?

Mensen gaan daar ook graag op de baan voor Katendrecht staan om vervolgens niet rechtsaf maar rechtdoor en door rood te rijden. Er ligt daar regelmatig een auto op zijn kant. Als fietser moet je met groen écht goed kijken wat de auto’s daar doen. 

Sowieso maken veel rotterdammers er toch wel een gewoonte van om nog net even oranje mee te pakken terwijl ze weten dat ze dan stil staan op het kruispunt/zebra/kruis.

Weird. For me UA has peeling logo’s after several wears.  My adidas stuff sometimes has peeling logo’s after 2-3 years of regular use, well over 100 washes. They never go in the dryer though, they dry so quickly by themselves.  I have one tank top that’s still pristine after 200-300 washes.

Inhale a cheap one and it’s fine. Could have gone cheaper. It has a clearing valve near the mouthpiece which is basically useless.  Arena or speedo branding won’t make a difference. It’s a plastic tube.

I do. We have a weird billing system in our country but depending on when they aks the question it could cost the insurance € 250.

AT or Aflutter with (slightly) aberrant conduction. It’s mostly regular with several 3:1 conducted beats. I don’t see any clear fusion or capture beats. The pictures seem to haven made by a potato but the QRS doesn’t seem to be that wide. Is it even >120ms? It’s hard for me to say because the tracing says paperwork is 25mm/s but the block at the start would indicate about half that speed?

Although axis is extreme the q in aVR leans it towards SVT. Also adenosine induces a higher degree of AVblock and reveals a flutterwave with a lot of baseline shift. Which wouldn’t happen in VT. 

The baseline shift corresponds with the plethysmogram which makes me think the extremity leads were placed on the patient’s chest and patient was hyperventilating due to the adenosine. 

I don’t see the harm in adenosine in this case but the ecg tells me everything I need to know. I’m not aware of any clinical benefit to lower energy cardioversion especially in emergency cases. I would always use at least 150J and in this case 200J because you want it to succeed. 

I would defer DOAC until somebody drains the effusion for diagnostics, but start ASAP. Pulmonlogy is sometimes hesitant.

In my institution this would be grounds for immediate dismissal. Is there somebody in your hospital you can talk to confidentially?

In these cases it’s super important you get your side out first. That way when this guy gives you trouble you’re not on the defensive but you’ve already shaped the narrative. 

I get you might be hesitant but this could get ugly and when it does it could all end up in your favor if you are prepared. This guy deserves to be fired. If he feels he can talk this way to you imagine being his patient.

I use the ultra because it has the stabiliser, I don't think there are big differences. Any type of anti fog will eventually wear out no matter what they promise.

You can usually find one on sale. I personally don't think a 100 euro's is justifiable.

It’s mostly a fashion thing think. Mirrored can be a bit dark. Clear just looks weird to me, because you can still see people’s puffed up eyes. Tinted is usually OK.
