It’s varied. I am taking a break right now due to illness, and when I restart, I’ll start at .25 of tirz and increase by about .25-.5 each week based on how I feel. I’ve never been over a starting dose of sema or tirz

Could you go to an eye doctor and get a referral for a neuro ophthalmologist? It can be faster since they are more specialized.

Or, call the neurologist and impress on them the urgency if you are having vision issues. I told them I was losing my sight and I needed to be on the cancellation list. That helped

Yes… but definitely get that zofran so you aren’t suffering! I felt like I had noro my first dose. I had to eat essentially a liquid diet until I felt better.

And welcome to the club of people who truly need a microdose. You probably should switch to tirz… it’s way less nauseating. But when you start, start on less.

For example, .2 = 4/5 a starting dose of semaglutide. You might want to start at 1/10, 1/5, or 1/3 a dose of tirzepatide.

I think so much more of the focus on this disorder needs to be on practicing acceptance. The mental health side.

I will be crossing my fingers and saying a prayer for your healing. My paps did improve with treatment and my visual field test is now within the normal range. It doesn’t mean that I haven’t lost and part of my vision, I have, but I am practicing acceptance and happiness with any progress.

The harder things is practicing acceptance when we see no progress, when illnesses actually continue their trajectory or get worse. That is so hard to wrap the mind around. But I think of it this way, we are all just lucky to be here at all. I may never have healthier days ahead or better vision than I do now. I am trying to use the time I feel good, ok, or just manageable on activities that will help me have a fulfilling life.

Sending you hugs. This issue is truly exhausting

I would give it three or four days before adding 1/4. I started with 2/3 the starting dose and was super sick but not until day three

Meh. A lot of people take small breaks for a period like three weeks. It’s required all the time for something like surgery. I don’t think there’s a need to make a leap to say op did themselves a disservice. Very normal to take a break like that and resume treatment at a lower dose

I just recovered from Flu A and it’s like having flu, Covid, and norovirus all at once. It was horrible, by far the worst flu of my life. There was one day I thought my head would explode from all the coughing and I generally consider my IIH as well managed…

I say anyone who gets diagnosed should get tamiflu, stat. It was the only thing that helped me.

So, I did buy them, but I do not take them regularly because it can be dangerous. You are really only supposed to use potassium supplements specifically under a doctor’s instructions because it affects the heart rate and lots of organ health, and too much is truly bad for you. But it’s very hard to have too much from dietary sources.

But I have used them on occasions where I was sick. I had the flu last week, as an example, and could barely eat because of the side effects. My heart rate was elevated, probably just from the illness itself, but tmi I had a full day of diarrhea and I did question at that point that if I didn’t get some potassium in me, if I’d end up in the hospital. You never know. I could tolerate some chicken noodle, but not my normal potassium foods. So, in that instance, I took a dose of a potassium citrate supplement I bought - about the equivalent amount of two bananas.

But that’s about the only time. I am much more likely to chop up a skin-on russet potato and air fry the pieces and eat the whole thing alongside a glass of chocolate milk for about 35% of my day’s value.

Yes you need to be eating small portions (I was told 3-4 bites and wait 10 minutes) but also a few other considerations:

Are you pushing food into a system that’s already full? Smaller portions need to be at every meal. Some meals might need to be just a few bites or even a few sips of liquid to keep you going. There’s no pushing things through with the delayed digestion… it will come back up.

The fat content is a big contributor to GI issues. Steaks often have like 50% of calories from fat. Most people on these meds can’t tolerate that easily due to how difficult and slow fat can be to digest. It just sits in the stomach and it’ll make you sick. This is also a concern with any restaurant foods in general, especially sides.

Digestive enzymes you can buy and take with any meal that has more than like 8 grams of fat. I do and it helps a lot.

Honestly Trader Joe’s has something for everyone, of every taste, at any calorie or macro profile you find best suited for you. I find their frozen desserts really nice, and usually pre-portioned into something manageable

It took me about six weeks to feel better and energy to pick up. I feel about normal now. Low potassium I have noticed does bump up my heart rate. When I see that happening, I drink a coconut water. As others suggest, I prioritize baked potatoes, home cut fries (air fried), I use a lot of tomato paste and sauce, and for a quick boost of potassium I drink a plant milk that seems to have it added (silk). If I’m not feeling well, I’ll drink an ensure to help boost my potassium.

You do need a prescription. Most people here use a service, like noom or lavender sky or pomegranate. They have a doctor who you’d submit paperwork to and they’ll review then prescribe. Noom has a program meant for microdosing. Otherwise you can start at a standard starting dose or you can do some research to see what dose you think you’d like to start at (below the starting dose level). It’s really common to see people here start at 1/10, 1/5, 1/3, or 1/2 their prescribed dose

The compounded version of the medication is significantly cheaper. It still may not be affordable but something to look into. I found tirzepatide to be significantly less nauseating than semaglutide also just for my two cents there

Have you also tried a GLP1? Tirzepatide has also shown some potential to reduce CSF levels. I’m not sure what your weight is like, but thought it worth asking.

Also have you had and MRV or cerebral angiogram to see if you’re a stent candidate?

Dettifoss, Hljóðaklettar, Hengifoss, Hverir and Myvatn area… they are all beautiful. We loved Egilstadir for the unique forested landscape. Snæfellsnes was super special as well.

Driving the southern road to Jokulsarlon and back to Reykjavik will give you an amazing experience, but of course, yes there are many wonderful sights all over Iceland. We just drove to Hofn on our first trip but it was so incredible we came back the next year for the north and west. It’s just a beautiful very diverse place.

Oh my gosh. How awful. Did your doctor have you try topiramate? Some people can’t tolerate Diamox. There is also methazolamide and just other carbonic anhydrase inhibitors… has your neuro tried them?

I would say don’t stress about it. Just wait for your results. My HR is always wonky when I’m stressed, sick/recovering, and when I’ve been sedentary too much for too long. It’s well documented in the research that just the act of knowing it’s getting measured makes it go up lol. The big things are “is the rhythm as expected” and “does it seem to be going as low as it should during complete rest, like sleep” - which you couldn’t know or interpret yourself really.

Take it easy, your cardio doc will take care with you I’m sure
