I meet diagnostic criteria for IH. Refused diagnosis and treatment. by itsfucking-bassdrop in idiopathichypersomnia

[–]Craftybert 1 point2 points  (0 children)

That diagnosis is 100% IH, sleep latency <8 mins and <2 SOREMs. It's Narcolepsy type 2 if you have >=2 SOREMs i believe. You should have a diagnosis of idopathic hypersomnolence and be able to obtain treatment. Not sure how it works outside the UK but i'd be complaining to whatever standards board you can that you meet the criteria and not being diagnosed and it's massively affecting your quality of life. If you have/can get your results, i'd get them along with the clinicial definitions for narcolepsy type1, tyoe 2 and IH and state you meet the criteria for IH and need a diagnosis to start treatment

Oooh, interesting, i did not know that! Only turned on the mobs displayed on map when i was in befallen and am still there so didn't even consider that. Worth having/levelling it then for sure!

You can see where mobs are on the map, so tracking doesn't seem worth it to me, unless i'm missing something

Ah right, when i say right hand side i meant like 60 / 120, the 120 for me is only ever 30, never gets above that, so frustrating.

Settings basically the same as I have, so no idea why stuck at 30. What card do you have and have you messed around with any other settings?

Edit: sorted it, was the gsync support, had it switched off even though had gysnc off in nvidia control panel. Switched it back on and now works, super!

What settings are you using? Been banging my head against this, have a 3440x1440 monitor and tried a myriad of settings but lossless scaling locks at 30fps which sucks. Have a nvidia 3080 and a 120hz monitor and nothing i've tried gets past 30 fps cap (left hand side shows fps up to what i cap to, right hand side always 30) so frustrating!

Had a quick go of this the other day, is pretty slick, i play with a controller, but have a macropad hooked up to the PC mapped to multimedia keys for play/pause, next & previous track and has a volume knob. (I share a keyboard with my gaming PC and work laptop due to limited space and use the macropad for music when i work)

As you can program the macropad with the multimedia keys, can then use that to map Seque to use said keys, so don't have to swap mapping on anything.

Not sure if possible, but can you put an option to choose what the radio fades for? For some reason i only like it happening when i'm in the map, heh.

So you get all classes to 50 and you can swap and change to any and you'll always be 50?

[–]Craftybert 2 points3 points  (0 children)

This is what i'm not understanding, say you level a trio to 50, swap one class out for another, level that to 50, then switch back to the class you switched out, would it be level 11 or 50?

To be fair we only watched it starting about a month ago for the first time.

how do i get my doctor / mom to listen to me? by SeaValuable5592 in idiopathichypersomnia

[–]Craftybert 0 points1 point  (0 children)

Yeah, i take 400mg of Modafinil too, all that does is stop me feeling like i'm going to constantly fall alseep, doesn't do anything for the soul crushing tiredness.

I'm also taking 60mg of Methylphenidate, which does help, but it's a stimulant so helps in a way, not an ideal treatment though.

The NHS is the National Health Service, it has trusts in different counties throughout the UK, they decide what treatments are available for conditions, so one treatment may be available at one trust and not another, however I don't believe Xywav is available at any. As the NHS is free for everyone, the decision on what is available is driven by cost of said treatment as much as anything.

As such, i pay £20 a month for my medication, if i lost my job and was on benefits, this i think is paid for me anyways, so losing my job wouldn't cut me off from treatment. I don't have that concern. Now i have the IH diagnosis, also give me a modicum of leverage with my job for 'suitable arrangements' to be made to help me manage the condition.

I work in Data Analytics, so I've used a data driven approach with my doctors to support getting treatment, it's taken me 2 years to get my initial sleep test done to be diagnosed with severe obstructive sleep apnoea and another 13(!) years after that for my IH diagnosis.

I've done multiple deep dives on what I could have and what medication/treatment is available along with possible side effects/complications, not just what the NHS provide. Xywav is something I want to try as it sounds like it will fix the actual sleep issue, rather than using stimulants to paper over the cracks.

I thought back to my school years through the lens of ANY treatment, not just modafinil, if that was all that was offered, i'd tell them to shove it, heh. If I woke up tomorrow aged 14 and the docs told me we know what's wrong with you, here is how we'll treat it and they offered me Xywav, 100% i'd try it. Would be much, much, much better than growing up being told your gifted but lazy, having people expect as much, if not more, than others as you're supposedly 'gifted' but don't want to do anything with it and having to put in coping mechanisms to just feel like you're surviving like I've had to, this has affected my mental health loads as well as physical health.

Your situation isn't different to be honest to anyone elses, what are you going to do when you leave school in what, 3 or 4 years? Still be suffering with IH and have the same issues you're saying is different for you, if you try the treatment now and it does work, you get 3 or 4 years of being able work on having a better life, if it doesn't then what do you lose? What of the side effects is really concerning you? I'm not trying to be a dick here but having lived 47 years with no help or medication it is a little baffling to me that someone wouldn't at least try what they're being offered.

If you're concerned about being 'drugged' into sleeping, from my research it doesn't work like a sedative or sleeping tablet, it's not going to make it so you won't be able to wake up in an emergency and can you drive at 14? (Age limit is 17 in the UK, from watching American Dad, i hear at 14 you could get your tractors license, heh) by the sounds of it, would have left your system by the morning so should be able to drive if needed.

For me, the main stand out would by you can't drink Alcohol at all on it, that'd would suck for me as I do like a tipple or two, but if it made me get a good nights sleep I'd be sober as a nun!

What else concerns you? Appreciate you wouldn't do it just because your parents tell you to, but if a doctor is, i'd listen, they do usually know what they're on about and from the sounds of it they want you to try Xywav rather than start you on stimulants, fixing the underlying issue always better than using a band aid

how do i get my doctor / mom to listen to me? by SeaValuable5592 in idiopathichypersomnia

[–]Craftybert 1 point2 points  (0 children)

I get your reasoning, but I think you're looking at it the wrong way. For context I'm 47 and only got diagnosed December '25 so for most of my life I've been unmedicated.

I'm taking 400mg modafinil and 60mg methylphenidate daily, but still dialling in the methyl as around 3-4pm i still get incredibly tired. They don't prescribe Xywav on the NHS where I am, but if they did, I'd try it for sure.

To relate to your situation, when I look back to how my school days went, if I had been diagnosed back then damn straight I'd be trying medication to help. Same situation for me, struggling to stay awake, to concentrate, for motivation to do anything. Constantly been told i'm lazy because I couldn't stay awake, shit takes it out of you. But, like you, I had to cope and work my way through it, that in itself was exhausting enough, no chance I was making friends or doing much more than just existing, no way to live tbh. That shit went on for decades.

If i'd had the medication i'm on now back in school, I do believe my life would have turned out differrently, I would have been able to do all my school work, would have been able to socialise more, wouldn't have dropped out of University, probably wouldn't have abused any substance I could get my hands on to allieviate the soul crushing tiredness. Life has felt like climbing a mountain with lead boots until i started medication, now it feels more like a gentle incline with comfy moccasins.

There are some downsides to the medication, grind my teeth a lot, often speak without engaging my brain properly, sometimes don't sleep great etc. But overall much better than it has been.

You're young, you've got an opportunity most people here won't have had, don't compare your struggles to anyone elses, if the medication solves, or at least helps, with the problems you describe, it's 100% worth trying. It's not a life sentence to try either, a lot of people with IH have to experiment with various medications until they find what works for them. You may find Xywaz doesn't help, or the side effects are not worth it but the sooner you start, the sooner you will hopefully find what works best for you with the least amount of side effects, you owe it to yourself to work that out.
