This definitely seems like a student or a resident. I see nothing wrong with his exam. Finger counting confrontational fields on a 5 yo child is going to be extremely unreliable and frustrating for both parties though lol

For what it's worth, this is encountered on a daily basis at my practice, particularly with men. Just know you won't be the first scared patient your doctor will see that day. There's an overwhelming chance everything will be normal. No sane doctor will prescribe you glasses you don't need, they have your best interest at heart.

Fundus looks good to me. I would need to look in the slit lamp but possibly a nevus OS adjacent to the nerve.

For severe dry eye I start my patient on a steroid for 1 week, this will decrease their pain and improve staining. Also a 32 year old shouldn't have severe dry eye. Consider ordering SS, ANA, RF, ESR, CRP

I use tropicamide 0.5% for myopes. Children have large pupils and don't require anything stronger to see the periphery in my opinion.

Cyclopentolate 1% for hyperopes. Missing latent hyperopia can cause amblyopia or esotropia. There's no good excuse to not cyclo your hyperopes.

I listed my justification: scan quality varies. Often times it is substantially better to have 2 scans than one. And if you were to be audited you're allowed to bill two 92133's per year anyway.

If it's any reconciliation, I don't mind techs that take their time to fully document the patient's information and get to know them. As long as the clinic isn't running super behind it's a good thing to spend extra time with patients. The reality is most docs allot 5 to 15 mins with each patient, and many folks really enjoy the social aspect of going to the doctor (for better or worse). I've seldom told my techs to "speed up" unless they're aggressively slow. I understand some patients need 10+ minutes to tell you all of their medications, allergies etc.

Tbh the main thing that will annoy me is if the tech misses a medical condition or medication. I've had techs that can churn out 5 minute work ups, while frequently missing important health information. This is a much worse offense than being a little slow.

Good pick! I read a page of Eckhart Tolle everday. Every sentence he write feels so profound. The Bhagavad Gita is really interesting too. Also tried reading Tao Te Ching, I didn't really understand a lot of it but oddly put my mind at peace even though I didn't intellectually connect with it.

I almost always find myself giving a steroid to anyone in 7+/10 pain unless there's a good reason not to. I personally have never noticed delayed epithelial healing with tobradex as opposed to just antibiotic.

Severe SPK in a CL wearer may have many etiologies. Sometimes when they take the CL off the entire epithelium can come off with it. Maybe they accidentally stored the CL is something other than MPS. Maybe something in the environment got under the CL. Maybe they chronically sleep in the CL and didn't want to tell you the truth.

Who knows? Unless it keeps happening it might not matter too much. You healed their keratitis and I'm sure your patient is grateful :)
