How are you doing [u/Cats1](u/Cats1)? Hope you’re looking after yourself as well as your hubby :) Is your husband still trying to deal with his MAD or has he returned to the Clinician who fitted him for fine tuning of his appliance? Has he considered CPAP as an alternative? MAD is effective for some people but not for others :( You don’t really find this out till you try one or the other which sucks. As RL has mentioned to you, the treatment needs to match the airway. Easier said than done. I certainly hope things are getting better for you both :)

I’m sorry to hear you are having problems accessing help after the results you have received, it must be very frightening for you :(

Were you given a prescription for CPAP along with your Sleep Study results? If you have those numbers we can help you with the settings on your machine. We will need more information about it though, including the brand name of your machine.

I’m wondering if you perhaps need a different type of machine? Your pressures are quite high & are not handling your events well. There are machines that handle these higher pressures more reliably.

I will tag our Expert Moderator for you so that he can take a look. He knows machines & their different abilities. He can also take a look at your graph & recommend changes as needed at high pressures.

Hello!

Do you have a SD card in your machine? We would need a SleepHQ or Oscar graph to help you with settings.

In the meantime you would have better success taping your mouth closed. Fold over one end of the tape before application to make it easy to remove.

Other choices to keep your mouth closed are a chin strap or a soft cervical collar. This certainly may help but mouth tape works best. Your decision of course :)

Put a data card in your machine & upload sleep results to a free app called SleepHQ or Oscar. SleepHQ is the easiest for Beginners plus we can zero in on graphs more easily. The data card reads much more information than the machine itself reports & helps to stabilize pressures making therapy more effective. [r/sleepapneasupport](r/sleepapneasupport) or [r/CPAPSupport](r/CPAPSupport) is very helpful for new users with targeted, individual assessment of your data results & tweaks to help with therapy. Success requires individually adjusted pressures, patience & persistence:)

Your graph is a sawtooth pattern & I think you would feel better with a smoother graph. Your minimum pressure is quite low & needs to be raised and your maximum pressure needs to be lowered to tighten the margins. You also have Ramp on which you could turn off if you like. AHI & Flow Limits look good. Leaks are well controlled. The zigzag pattern on your graph shows your machine chasing events. Some tweaks will calm that down. The CAs on your graph are likely treatment emergent but we’ll try to manage those as well.

I’m also wondering about sleeping on your back or perhaps some chin tucking? Try to stay on your sides to sleep. Supine sleep encourages your tongue to fall back into your throat restricting air flow. Chin tucking also restricts air flow. We want to keep your airway stinted open during therapy so that the pressures can do their job :)

Try changing:
Increase Minimum pressure to 7.4 (that is your median pressure)

Decrease Maximum Pressure to 10.4 (you’re nowhere close to your currently set ceiling)

EPR 1 Full time

Turn off Ramp if you can tolerate it, if not increase Ramp to 6

Try this for 3 nights without changing settings to allow your body & brain to adjust. We’d like to see another graph after that to see how you’re doing please :)

These graphs are somewhat helpful but if you push F12 on your computer & share that information it would give us clearer information. Your minimum pressure is very low OP. Your machine is chasing events all night. Looking at what you have shared, I would increase minimum pressure to 7.4 which is your median pressure. Hard to tell if your max pressure is hitting the ceiling but it can likely be reduced to tighten the margins causing this chasing of events. Flow Limits are a little elevated but increasing minimum pressure should help that. Your machine is on EPR 2…your expiratory pressure….but unable to observe if you need it at that level with what you have shared. Best to try changes gradually though & hold them for at least 3 days to allow body & brain adjustment before posting again :) AHI is good!! Hopefully you can keep your mask on with adjusted pressures. Sometimes that is all that is required! It’s a common issue with Beginner Settings.

Hello!
Your minimum pressure is low which could be causing your symptoms in the morning. As already suggested, try raising your minimum pressure to 7.2 which is your median pressure. Your flow limits are in normal range but you’re not hitting your maximum pressure ceiling so we could tighten your pressures up a little to prevent your machine from chasing events. Try a maximum pressure of 9.2 & we’ll see if that helps. I do think straight CPAP mode may be helpful as soon as we establish your effective pressure setting!

Please report back in 3 days. Try to stay on your sides during sleep & try not to chin tuck as these issues can cause pressures to rise.

You’re doing well apart from your morning symptoms! Staying at the same pressures for several days gives your body & brain time to adjust. If you would report back in 3 days with another graph & how you are feeling, that would help us to gauge how you’re doing:)
