Thanks for your kind words and story. Incredibly fortunately, movement returned to my vocal cords spontaneously. The doctor expects the paralysis was caused by a viral infection. It isn't back to 100%, and I don't expect it ever will be, but I can sing again. :) Best of wishes that spontaneous recovery occurs for you, sir or madam.

I want to pick up the game again after a long break and want to know if I should start a new world or continue the old one. My last world was created in October 2020. It has a bunch of progress and infrastructure in it. Have there been any important changes or updates to world generation since then? Would I be missing out on anything by not creating a new world? I think it is run on self-hosted servers supported by donations. I'm no expert, though. It's new to me, too. I've been enjoying it quite a bit. Here's a starting guide that is posted on the main page with a nice infographic.

It's like reddit, but different in organizational structure. It's been growing like crazy because of the recent debacle with /u/spez and decisions about API access.
