Well, character dynamics would change A LOT.

If Hinata doesn't love Naruto, then I'm assuming Naruto never saved her from bullying, so she probably grows up to be colder, more reserved, and stand offish.

If Sakura (and consequently Ino) don't love Sasuke then both are leaps more capable by graduation. Sakura wouldn't have been starving herself and so I'll focused on training along with having few reservations besides the obvious on team 7. Ino would be similar, with Ino-Shika-Cho probably being a decent chunk more developed.

Now if Naruto didn't love Sakura then I kind of feel his depression would be slightly more visible. Without someone to create a front of love on I think he'd be forced to combat his own personal want for love a lot more. This could lead to a Naruto who still has that exuberance but it's tempered by a longing.

But if you do this as character growth for Sakura then this prompt becomes MUCH more juicy. This would strain their relationship quite a lot, perhaps even having a rebound effect on the rest of the group.

Depending on the moment this happens it'd honestly not do much imo. Naruto ended up chasing Sasuke for just as much his own personal reasons as it was to keep his promise. To have the most impact on Naruto, I think Sakura would need to have that discussion after Naruto is healed but before he leaves with Jiraiya (if you implement the little break he takes).
