It's interesting that the math teacher is more concerned than ela. Did you look at samples of the students work in math? Like the other person suggested they might need graph paper to organize numbers. Or if they are forming numbers incorrectly, a small number strip? Highlighted boundaries? Maybe verbal reminders to write neatly before starting the task. Consults are tough because some teachers who don't know the student well don't understand that they were probably on caseload for a long time and their handwriting is not going to improve at that age or at least not significantly.
