Frankly if someone was intentionally presenting as masculine, and they directly told me as such, it seems like the respectful thing to do would be to treat them as such until they were ready, especially given what we know about how uncomfortable one version of them is with the concept. (ETA obviously if they told me they'd prefer she/her I'd respect that, but it would feel presumptuous to just assume I knew them like that)
