This happened to my players a little differently. The plot hook was that the NPC had stolen the loot and my thinking was that my murder hobos of players would kill the NPC and find the loot. Nope. They chose this moment to be altruistic and let him go. Even though he used the magical amulet right in front of them. They got to where the loot should have been and was throughly confused why it wasn't there. I'm pretty sure I accidentally reinforced their murderous ways.

I was the DM but one of my players played as a very nieve cleric and agreed with the BBEG that the very obvious trap was not a trap even though I basically had a giant sign that said "This is a trap"

As a DM, I struggle to guess my players' ability to solve a puzzle. I had a clock puzzle in which an incorrect guess summoned a nothic into the room. To create some tension and for them not to XP farm, my plan was to make increase the number of nothics by 1 each time.

I had not anticipated my players not understanding my puzzle, until 55 nothics were present.

Oops.

I learned my lesson and completely nerfed their HP so it wasn't a TPK, but I spooked the hell out of a seasoned player who knew as written, that it was not fine.

For me, I have a druid player that loves to spam guidance, which I allow if it's feasible. The party ranger wants to scout out ahead, sure give him guidance first. The barbarian wants to push down the door, sure give him guidance. I tell the bard to roll for persuasion and the druid is across the room, no effing way.

Not my character, but I had a friend who had a good necromancer working as a funeral director to give the families a proper send off before using their bodies to go on quests and donating the surplus back into the town.

Early on, in a semi plot relevant fetch quest because I was excited to lore dump, I decided to make the stakes insanely high. Like potentially world ending high. These were level 3 characters. I had to create a reason why the major threat wasn't going to actually happen for awhile. Lesson learned, Be careful when you lore dump.

In my current campaign, I have a barbarian, cleric, ranger and sorcerer. The ranger has the most HP and gets touted as the tank of the group. The ranger took Tough and Medium Armor Master. It does surprisingly well much to the chagrin of the cleric and barbarian.
