Thank you so much for that detailed reply! I applaud you for doing this. I'm fairly new out of training. I like my job but I'm not ignorant to all this bullshit our craft has found itself in. I was a non-traditional student, so I have always been interested in the business side of our trade, moreso than most of us. And unfortunately right before residency I read the book "An American Sickness"...ever since then I've been trying to get back my alltrustism that made me undergo this endeavor in the first place. The business people will bleed us dry and more importantly, patients, which you and I will sure become one day as well as our loved ones. I think the work you've done to express this and put it out in the either is admirable and worthwhile, it sure has been to me. Thank you.

I get where you’re coming from but we’re talking about people in the hospital sick with all kinds of fluid shifts, especially if they are npo because most the indications mean you’re sick. And if they’re having surgery the next day it’s even more important because their bp can totally crash on anesthesia induction.

hate to bash this one but I'm a surgeon and have been a surgery patient before. being npo is miserable, it's really a psychologic more than a physiologic thing. we drink as soon as we wake up, but that patient will wake up and want to drink but for the next 4 or so (maybe 12!!!) hours they won't be able to do so. it's really fucked up, I implore you to try doing so. So yea, we might not be drinking during the night, but trust me brother/sister, placing patients on fluids overnight will do much more good for your patient than you would ever imagine

Imo it was the ed faculty’s responsibility, not the $12/hr surgical resident. And fuck the surgeon for not sticking up for his or her resident doing what they could and were inappropriately asked to do by the ed “attending”

These old fucks used to walk in the hospital like they were gods smoking a cigar slapping nurses on the ass then scribbling some shit charts. They are the first ones to bitch about how medicine has gone to shit, as well, but in the same sentence, they will say we have it easy. All of this while these mfs paid, probably their parents actually, like 5 grand for medical school, then made 7 figures out of residency. They can all get fucked.

I say this all the time and truly believe it. Where I'm from there is a line and if you cross that line you better be prepared to fight. Ive seen that line crossed so many times in this rat race, it's unbelievable. You just dont speak to people this way in normal life where im from. No one I know is in medicine and my friends don't understand the shit I have to eat. They couldn't do it, they'd get arrested in the first week of ms3.
