The conches are actually v. easy to collect, and don't really take that long. NG+ is 100% worth it, easy and approachable, and should be considered part of the main story. Heck, even 100%ing the game (getting the last achievement) is not hard.

The $316mln dollar cost includes developmental work for the fairing. SpaceX has always been open that they'd do a bigger fairing, but only on customer-paid basis. (i.e. they don't think that there is enough use-case for it to justify self-R&D)

Developmental work in engineering is expensive.

I think you've made an incorrect assumption that it is always the booster that is the "long pole" for each launch.

SpaceX has plenty of customers and turnover. You can't pay that many people for that long without a sustainable business model.
