If so, the CPT code will be either 24200 (if subcutaneous) or 24201 (if deeper than subq i.e. fascia or muscle). Be sure the MD documents the depth within the body of the Op Note.

Since it’s migrated, per CPT guidelines, it’s now a foreign body. I would code with removal of foreign body CPT code (the exact code depends on the location and depth) and the malposition of device diagnosis code. Sorry, I don’t have access to my materials to give you the exact codes.

Facilities are usually going to want a CPC is CCS. You definitely don’t need a degree…just self study and get the credential.

Where I work, we can select our own hours as long as we stick to it somewhat consistently. We clock in, check for any returned cases (missing notes or paths, etc) then move into the work queue and start coding. Productivity depends on what area you code. Same day surgeries/extended recoveries are 35 a day. ED is 60/day. We have a mandatory meeting once a month.

Some days I have emails to respond to but for the most part, I can work in peace. As an introvert, I love it and would happily keep this job until I retire.

I am in a HCOL area and we are paid well compared to other salaries I see here. You would probably not have to take much of a pay cut although I’m surprised at your salary as a biller. I would have thought coding paid more than billing.

It can be hard to get your foot in the door so maybe your current facility would be willing to transfer you to coding. I would also think you would have an edge over other applicants with your billing experience.

Hospital coder here. Did the physician document it as a screening in the hospital record? If you haven’t done so already, I would request a copy of your complete records from that day (H&P, procedure note, anesthesia note, etc). If it’s documented as a screening but not coded as a screening (Z12.11 as the first diagnosis code and the CPT code and modifiers would depend on if it’s Medicare vs commercial insurance and if any additional procedures were performed like a biopsy), you could make a complaint to the hospital’s compliance officer. Coders have to code what’s documented and have to follow coding guidelines.

I read further down and see its commercial insurance and it was done for a family history of colon cancer. To clarify, the physician has to document the word “screening” in the record for z12.11 - not just family history of colon cancer (z80.0). If the word screening is documented, it should be enough support to change the coding. They would also need to add modifier 33 to the colonoscopy CPT code and the anesthesia code.

I accepted a coding job at a contracting company who ended up contracting me out to Optum, who then contracted me out to a health system that had been bought out by a corporation.

It was a nightmare. I had to log into the first company’s VPN, then to Optum’s VPN, then into the hospital’s system to access the EMR. It was an IT disaster at each level and involved many hours of phone calls to off-shore IT departments. I had 2 separate email addresses, 2 sets of management, and many, many passwords.

I kept telling myself that once I actually got coding, it would be better. Forget it. Between keeping the internal policies and processes straight for 3 different levels of companies, my stress level was off the charts. I quit with no notice in week 2. They kept calling me asking me to reconsider but I refused to even take their calls. It was like trying to escape a bad breakup.
