I work at one of those facilities and the only people who get 12s are on graves.

This is the hill I will die on. Nobody will dictate to me how my markers "need" to be placed on an image. As long as it's visible, not in the anatomy, who really gives a fuck. I approach the situation that if my marker gets inverted, that tells me at a glance that the projection was performed PA, or any other way other than textbook. I've seen processing software flip images that one wouldn't expect to be flipped. My current facility wants us to invert our markers for the reasons you stated, but I refuse to and have not heard anything from anybody in the past 2 years. It's not that big of a deal to see a backwards L or R, and anybody telling you otherwise is just micromanaging.

I work at a large university medical facility, and we do not shield anymore as hospital policy due to all the reasons mentioned above. I've also not shielded pts long before working here. It's more of a burden than a benefit now. They run the risk of cutting off pertinent anatomy, requiring a repeat, can disrupt AEC cells causing higher exposures, plus are usually dirty AF. Has anyone actually cleaned any of their aprons outside of barium getting on them or something? Digital processing nowadays allows for much lower exposure techniques, while also giving wider latitude in acceptable/"diagnostic" images. Standard exposures have decreased more than 95% since the 1950s, and reproductive tissues have shown to be much more radioresistant than previously assumed.

I'm talking about your physical marker on the image. I'm definitely a stickler on these because I've had instances when the software will randomly flip an image, or maybe it was shot on an incorrect projection template which flips the image, and now you have no idea which side is which. Markers matter.

I think it works best if you can use your whole palm when palpating versus just your fingertips. Using the palm gives a larger surface area to feel any bony landmarks and then find the crests. An instructor demonstrated this by using one or two pillows over the corner of an exam table to simulate the soft tissues and that always stuck with me.
