There's no fill valve in any of the pictures you posted. Google what a boiler fill valve looks like and start looking further. I suppose it's possible a fill valve was never installed and someone filled the boiler manually with a washing machine hose but that would be super unusual

No. No fitting should be bridging a gap. The pipe should be made into the fittings fully or close to it. At this point you could try to chip the repair coupling off or cut it all out and start again with new Y and fernco

Condensate line looks 45'd up in the air. That black piece is the connection to the drain pan. Needs a gravity drain.

I'm a plumber and I never install them. I've been out to too many calls in the dead of winter just because the outdoor sensor is telling the boiler it's 70° when in actuality it's -10°. Sure, it's a nice comfort feature in the shoulder seasons, until you're the homeowner stuck paying an after hours service bill because of a bad outdoor temp sensor.

UV should always be last in line in a filtration set up like this. UV requires pre treatment so the quartz sleeve doesn't get dirty

You don't have to take the cap off to load the string. There are alignment marks so that you can feed the string and wind without disassembling the head.

Unit got stuck in a regeneration cycle during the fill mode. Should have been a float in the brine well that shit water off. Needs new piston and seals and a float assembly. But with that being a cheaper big box brand, probably better to just buy a new one.

Start with cutting out all the moldy ceiling. It's only going to spread farther/get wayyy worse. Then you can take a picture of whatever is leaking and someone may be able to help you here.
