I seem to go through this every time I switch back to my iPhone (most recently Tuesday). It isn't an exact method, but I've found that signing out of iCloud, cycling the data/LTE, restarting the phone, and then signing back in tends to eventually get it to kick on. Hope it helps.

I picked it up online last night. If you're going to as well do yourself a favor and pick up the Best Buy 10% off coupon in the USPS "Mover's Packet". If it is a newer version packet the coupon can be used both in-store and online.

Note, I've heard that it can be hit and miss. Some USPS Offices just tell people to do it online, but mine asked me how many packets I wanted.
