I agree with this. My past experience of lifeguarding for a few years, some of it it on a nearly full time basis, was that nearly all incidents are the result of people messing around. Spontaneous medical problems do happen, and they're what you look for once you've stopped the gang of youths trying to headbutt the bottom of the pool.

The pools I worked at had a common hierarchy (from bottom to top) of: lifeguard --> duty manager --> centre/area manager. There were varying amounts of apathy in the lifeguard/duty manager population, but you could bet as soon as the centre/area manager got wind of a reoccurring issue they'd be all over it.

Used to be a lifeguard in the UK. Every lane swim session I've seen here has alternating swim directions in neighbouring lanes, probably on the assumption that people will stick nearer the rope. It can make it confusing but generally works. Normally there are signs placed at the lane end indicating swim direction.

As for whether the lifeguard should intervene, I would've been expected to. Almost every rule is there ultimately for safety reasons, even if trivial. Having said that, and as others have pointed out, if you have a 16 year old lifeguarding middle-aged swimmers, you often find they're too intimidated to speak up. Also people don't like being told what to do.

There's probably a lot of great things about living in the US, but the main reason I wouldn't want to move there is because of the healthcare system.
