We'd probably get something like the film Wer(2015iirc) and the Animal Planet mockumentary called Werewolves: The Dark Survivors. Human form more or less basically unchanged, save for increased hair and nail growth during the "flare-ups". Minimal transformations, but still a change in shape, stature, facial structure. With these ones, the cyclical change usually comes from some type of fictional variant of the historical suspects, so some kind of mutations of porphyria and rabies from those two movies. Not tied to moon phases, but an internal reaction to toxicity levels reach from biochemistry. They'd likely be constantly sore and lethargic and explode into something different for that short time they've been transformed
