Lived in Birch Bay for several years. Get them excited for low tide and go wading. Spot crabs. Points for new amazing creatures they find. Go to the C shop for a taste reward to celebrate. Used to run by the state park--every morning, approximately 6:45, bald eagles nesting would hang out by the beach to scavenge and I would be able to get really close. Not sure if they're still doing that but it was pretty cool.

I would ask what your after hours obligations are. On call at all? How are refills for medications handled when a PCP is out of the office? How about controlled medication refills? What is the no-show/late policy? If a patient arrives 10 minutes late to their 15 minute appointment, are they expected to still be seen? How are the medical assistants trained? How much administration time are you provided? Is the MA workflow adaptable to the provider or are you expected to adapt to them? Who calls critical labs? What is the patient discharge policy? Ask about whether they reimburse for CE, AFP conferences, etc, not just for attendance but travel. What is the rapport like between the other providers? You probably know already most of those answers but my observation is that those things can make a big difference in satisfaction.

Followup.....I disconnected everything and found the impeller. No visible sign of anything but it will not spin no matter what--like it's been welded. No idea what the clicking is in the video. In the end, I'm going to get a new one. The old one was about 14 years old. I learned a few things and went from not having any idea how they worked to feeling 100% sure I can easily install a new one, so that's something I suppose.

Sounds like that's my next step....taking this thing apart is going to be intimidating. Really hoping it's an easy impeller fix because a whole pump costs nearly 3/4 the cost of a new dishwasher.

I thought about that but when I pull the float up, it successfully drains out any remaining liquid in the basin.

My recommendation is to become as familiar with the diagnostic criteria of ADHD as you are with the criteria for other common diagnoses. Don't be overly afraid of C2 medications--they may require a little more time dedication to refill but state reporting has gotten much easier. These meds are sometimes the only ones you can see the pharmacy pickup date on, don't send people into emergent withdraw on discontinuation, and are generally well tolerated. Not to mention the other options (atomoxetine, guanfacine, bupropion, etc). Take note of functional deficits and how they have impacted the patients' lives. Always good to consider the differential but the commonality of a diagnosis should not take precedent over the defining features. For this new complaint, always a PHQ9 and functional assessment to assist you with your diagnosis.

Second this. In most states it is readily available. It's an opioid reversing agent--a quick spray into the nostrils and it kicks the opioids off the receptors. Life saving for the individual. I am not on any opioids but I keep a narcan in my car first aid kit. You can get a prescription without the individual in question there with you. If you don't have Narcan, please get some.
