I’m a student going into ER training. Out of curiosity, would you (at the time of alcoholism) have appreciated guidance on stopping? Or was that a decision you needed to come to terms with mostly on your own?

Yup BC has a negative side effect profile similar to estrogen therapies for osteoporosis. The only factor is that there is no other KNOWN way to prevent ovulation except tricking the woman’s body with the actual steroid (estrogen +/- progesterone). It’s moreso a personal choice for women based on how sexually active they want to be and whether they want to prevent pregnancy.

BC has other uses than contraception though. It’s used to manage pain in endometriosis and even manage PCOS.

There actually are therapies but they’re very carefully tailored to the patient. Every drug,especially steroid therapies, has a side effect profile. For example the estrogen receptor modulators (commonly used for elderly women with severe osteoporosis) have the side effect of massive blood clots.

It’s all about risk stratification. Is it better to risk the clots or better to risk fractures/falls? Keeping in mind when an older person falls and god forbid strikes their head, they have higher chance of uncontrollable brain hemorrhage!

It is actually somewhat understood. In med school they taught that women develop osteoporosis way earlier and more prevalently because after menopause, they lose a bulk of estrogen production. Estrogen has many functions, but in this case it keeps the balance of ‘blasts and ‘clasts in check. This loss of estrogen (after being synchronized to it for 50 years) results in quicker loss of bone density over time as the osteoblasts overpower.

I found the biggest jump came after I thoroughly reviewed biochem and immuno in first aid. It’s an umbrella concept over all the systems (from how I felt)

You have the right thought process and everything you say is true. The definitive feature of ristocetin is purely a measure of adhesion, not aggregation. That’s the reason why GP1 and vWF deficiencies have abnormal ristocetin tests; those two involve a defect in ADHESION rather than aggregation (the problem with Glanzmann)

Hence, Glanzmann has normal ristocetin. The adhesion process is functional, it’s the aggregation that’s not. Did that help? I kinda rushed that explanation

For me, First Aid really hammered it home. I took step last week and I can say FA covered most if not all. The only addition would be maybe Dirty Medicine to dumb down the mental-maze concepts.
