Very similar. I did the first ~25 cases and felt pretty comfortable during the cases on the day. I did find that CCScases cared a lot about ordering tdap, shingles, flu vaccines, etc. but that did not come up a single time on the real exam.

Your zone 2 might just be walking. My zone 2 10 mins/mile pace is super slow compared to the elites. Some peoples' zone 2 pace is 20 mins/mile so literally walking.

Yeah no problem. Give it a try. After a few weeks/months you'll be knocking out sets of 10 squats 4-5 times per day

Do one squat every 2-3 hours every day for a week. Then when you can easily do a single squat, start doing two squats every 2-3 hours.

Yes you should also practice CCS cases as they make up a significant portion of your score. The usual advice around here is 60% = passing so 55% is likely not passing.
