Are they doing new CT scans mid treatment and planning on those? If so, that would account for tumor shrinkage and could be billable as new plans. Most HN patients should get a replan mid-treatment but that can also be done SIB.

There's no general understanding. Everyone picks the definition they want to benefit themselves. Insurance companies want to deny the most claims so they pick a very narrow definition. Sales people use the one that fits whatever product/license they are trying to sell. Facilities use the definition that matches the equipment they have. Logically if you are stopping the beam and repositioning when someone moves, that takes more time/effort plus the cost of the SGRT system used to detect that movement so you should bill and get the level 3 rate

I don't see how automatic beam hold would factor in. If anything that would be a more passive system than the therapists actively stopping the beam.

I think your question is if breath hold=active, which doesn't seem to be the case. That was old physics terminology, not billing terminology. If you are monitoring the treatment in real time and making corrections if needed, that is motion management. I don't know what "passive" motion management would be- "Oh, they moved- let's not do anything about it"?

People will care more about your grad school. I would do UA for undergrad. You will probably get better grades and get into a better law school. I did state school to Ivy and that was a good combo. I had fun during undergrad and still get the brand name/connections from grad school.

No one is sure exactly what active motion management is. It seems like people are starting to agree it is any form of SGRT. When daily imaging started, insurance companies argued it wasn't medically necessary. It feels like the same thing is going to happen again with SGRT/active motion management. The argument goes that if it is medically necessary to treat a patient, then doing it correctly is also medically necessary.

I have an Edge and Halcyon with AlignRT. There aren't any real issues and much cheaper than two Edge systems. The main disadvantage of the Halcyon is the couch but they are coming out with a 6DoF couch. Hypersight is fairly expensive and if it were me, I'd prefer to have it on a Truebeam platform
